A 36 C breast has approximately 1,000 grams of tissue. A 28 AA is less than 100 grams of tissue. This means you would be removing approximately 1,000 grams. In your case it would probably remove all your breast tissue and leaving you only with a skin envelope and areola. It is possible to do that but I do not think any plastic surgeon would do it. I do not recommend such a reduction. You are basically doing a mastectomy at that point. The result is often poor and the nipple may lose sensation not to mention the cosmetic outcome may not be desirable. Dr. Vasisht South Shore Plastic Surgery

Although it is certainly possible to reduce breast cup size from a "C" to a "A" cup size,it is not possible to give you good advice ( for example whether the operation is indicated and worth the downsides of the operation) without direct examination and a full communication of your goals and concerns. You will be best off seeking consultation with well experienced board-certified plastic surgeons in your area. When you do so, I suggest that you do not base your communication on achieving a specific cup size, as this can be a source of miscommunication and/or resulting dissatisfaction. I would be keen to know the reason for wanting such an extreme breast reduction. The only possible way to achieve a reduction of this magnitude would be to use a free nipple graft technique which would result in the permanent loss of erotic sensation.
