I don't know if my nipple is malformed or if I have two nipples? What can be done?

My left nipple is normal but the right isn't, it always was like this

it is hard to tell for sure without an examination. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Hi and welcome to our forum!
Your photo appears to demonstrate an irregularity at the apex of the nipple, which may be congenital or the result of breastfeeding. If of concern, the  irregularity can be excised, but there is risk of scarring of the milk ducts which may interfere with future breastfeeding.  As a result of the excision, the nipple will demonstrate a slightly reduced circumference. 
Visit a board certified plastic surgeon.
