Is it possible to remove a chin dimple surgically without implants?

I have a moderate chin dimple/ cleft and it’s been my biggest insecurity for years. I’ve tried getting it filled but it doesn’t really last long and I don’t like going back frequently to get touch ups so I’ve been looking into a more permanent option. I’ve started to look into genioplasty, however I’ve seen that most results are obtained using an implant, I don’t really like the idea of having a chin implant, so I was wondering if there is a way to just still remove the dimple without the need for one.

Yes, it is possible to remove or soften a chin dimple surgically without using an implant. Here are some options:

Fat Grafting: Your own fat can be harvested from another area of your body and injected into the dimple to create a more permanent filling effect. This method tends to last longer than dermal fillers.
Subcision & Fat Transfer: A minor surgical procedure where the fibrous bands causing the dimple are released (subcision), followed by fat grafting to smooth the area.
Direct Surgical Excision: In some cases, the dimple can be surgically revised by removing the deep fibrous tissue and suturing the skin for a smoother appearance. However, this may leave a small scar.
Sliding Genioplasty (Without an Implant): This procedure involves repositioning the chin bone to create a more even contour without adding an implant. It is typically used for more significant reshaping.

I have been performing subcision, a nonsurgical technique, for thirty years for elevating all kinds of skin depressions and indentations. Because subcision works by stimulating new, native collagen and new, native elastic fiber production, the results of subcision are typically long-lasting to permanent. Subcision can be combined with the use of fillers, not only for the immediate aesthetic benefits of the filler, but because certain fillers have been found to  enhance additional collagen stimulation. In our experience, we have had good success with permanently filling in a central chin indentation like yours utilizing the permanent soft tissue filler, microdroplet Silikon-1000. I don't think it would be possible for it to completely disappear, since it is largely caused by ligaments from the skin to the bone, however, it's depth may be reduced, when procedures are performed properly.
