At least from the photos and history provided, this appears to be what has come to be termed under-eye "puffy pillows." Typically these may result from an improper choice of filler for the infraorbital area and/or too superficial placement of the filler leading to lymphatic obstruction. They can be quite difficult to treat. I have been having success in shrinking these unsightly pouches in patients who have come to me from elsewhere using an algorithm consisting of intralesional injections of varying combinations, concentrations, and amounts of anti-inflammatory agents.
