Revision tip rotation/shortening of the nose - how difficult is this procedure? Can it be classified as a minor procedure?

aftHi. In March 2015 I had my primary rhino and Im pleased with the result, however Im not 100% happy with the lenght of my nose,it would suit my face to have a more upturned tip,a little bit 'piggy like' (I find such noses cute). In May this yr I shall have a revision,my surgeon will lift my nose tip without shortening of the septum (local anesthesia).My question is: is tip rotation a difficult procedure? what can go possibly wrong?can my nose bridge be ruined (i love my gentle slope)?

Yes, it is possible and very simple, an easy gesture trimming the lower end of the caudal septum makes the work.I have read attentively your message, nevertheless let me warn you severely AGAINST such upturned tip, since 99,99% of patients would find such piggy nose disgusting and grotesque, actually this is, in my vast experience performing revisional rhinoplasties, one of the most common reason for a revision (it is made by a technique called, obviously, septal lengthening).

turning up your nose while shortening your nasal length is easier to do as you can suture your cartilage under the domes (middle crus) back to your anterior septum; rotating your tip but not shortening your nasal length requires you to obtain support from another source which makes it difficult to do as you need a strong piece of cartilage which can only be harvested from your septum or ribs.

Depending on what is causing the nose to be too long, it can be straight forward or very complex.  The surgeon who did the first procedure is the best person to access the difficulty.
