I am 28 years old. The fat from my upper cheeks has moved downward to the corners of my mouth. OR the fat from my cheeks disappeared and the fat at the corner of my mouth is more apparent. Either way the shape of my face looks saggy. I have multiple filler in my upper cheeks and they still seem hollow on the sides and too full by the mouth. When I pull the skin back it looks much better. Is a facelift is the most effective option? Fillers don’t seem to help.

I will agree that proceeding with the nose surgery is a good option, and to feminize the nose, I would consider shortening the nose, decreasing the flaring of the nostrils and raising the tip-up. Furthermore, you can benefit from the Fat Injections to the face, especially in the area of temples...
I don't even buy into this notion of a tear trough. However most would recommend fillers. Do not have fat injected. It can turn into lumps that are hard to remove
