All my life I’ve been told I have a “depressed” looking face. I’ve noticed that I have hollow underyes and that my face overall appears very flat. How could I permentaly improve my face and make it look more feminine? (I’m a biological female, BMI is normal)

Hello, hope you are well. Cheek augmentation is a great way to proceed. There are different ways of doing this, but my preferred method is usually with pre-formed silicone implants. These implants are placed through incisions inside the mouth so there are no visible surgical scars. Unlike filler, they will hold their form forever and not dissipate or migrate over time. In your case, the goal is to increase cheek projection and fullness under the eyes, not to widen the cheek bones. I commonly perform this surgery under local anesthesia and the downtime is minimal.

The problem here is a very common one. The use of a robust HA filler cheek augmentation, properly performed by a skilled, experienced injector can not only contour a more feminine, oval, heart-shaped face, but create a smoother lid-cheek junction and more youthful under-eyes. There is no such thing as a permanent improvement from any treatment, be it fillers, surgery or gizmo lasers and RF devices. The simple reason is that the aging process continually leads to changes in the underlying bone, fat, muscle and "fabric" of the skin, which will necessitate further aesthetic work. It is the reason that despite the promises to the contrary even radical surgical facelifts and necklifts really don't give rise to permanent rejuvenation results. Ask anyone who has gone through expensive, invasive plastic surgeries five or ten years earlier how long any of their benefits actually lasted. You would be wise to consult with a board certified cosmetic dermatologist filler expert and best of luck.
