Hello and thank you for your question. After rhinoplasty (nasal surgery), the nose can become REALLY swollen, particularly around the tip, and this swelling can take a LONG TIME to resolve (up to 2 years to fully resolve). Also over the 2 years after a rhinoplasty, partially due to the swelling, the nasal tip tends to be a little more over-rotated than the final result will be. I would recommend that you continue to observe things for now.
Steroid injections can help with the swelling in some areas, such as the supratip. You are still only 7 months post-op, so ultimately you should still wait to see what your final results will be. Sometimes there are focal asymmetries and depressions which become visible over time as the post-op swelling resolves. My preference is to address these areas with dermal fillers, particularly if it is before the full 2 years post-op. Fillers are a quick, easy, minimally invasive, and very effective method for smoothing out any focal indentations to make the nose more symmetric and improve the overall rhinoplasty result. Of course they have to be periodically reinjected, as they naturally dissolve over time (usually over a year or so). The alternative is a full blown revision surgery, which is much more invasive. I constantly remind all my rhinoplasty patients how important it is to be patient after rhinoplasty surgery as it can take up to 2 years to see what the final nose will look like. I would not advise you to consider a revision operation prior to that time, especially if you are happy with the results achieved with fillers and after all the swelling has completely resolved. It depends on if you have any more cartilage in your nose. If they did a septoplasty, you probably don't have enough cartilage in which case you most likely need rib if you needed something as big as a spreader graft or a dorsal augmentation graft. However, with you only being 7mo out, it's possible that the tip is still swollen and may narrow more.
