Im 50, 180 Ibs. I was 155 Ibs 18 months ago but due to a thyroid illness I stopped working out and became house bound and ill. I had a jowl/midlift 10 yrs ago since then botox and a few filler appts. Last one in 2018. I always looked decent now I look like this. One side of jaw is more defined than other and when I am on zoom call I can see my lumpy face. I look like an overdone version of sylvester stallone. What can I do?? A lift? Help me please! Ive had referels and docs say just do filler. Thanks depressed.

When one undergoes consultation by a plastic surgeon, one's concerns should be clearly articulated to the physician and the physician will perform inappropriate exam in addition to asking numerous questions to determine if you are a candidate for a procedure. The photos provided are insufficient to fully address the question. At a consultation, it would also be wise to bring records from one's previous procedure including the operative note. This will also help a plastic surgeon at consultation determine what has been done and what can be done. We do age. Our 50s and 60s are times of more rapid changes as we go through the aging process. If one has a procedure at any age, the moment the surgeon stopped operating, the aging affect continues. It is not uncommon to undergo secondary lifting especially if one had a lifting procedure at an earlier age.

There may also be edema or swelling under the eyes (difficult to see in images) that may be a result of your thyroid condition. This is very difficult to treat if present. The best option for facial rejuvenation will depend on your goals. For modest, less invasive improvement, fillers or fat grafting to the mid face will help restore lost volume and create a lifted appearance. If you desire additional correction of the neck laxity and early jowls a secondary facelift with fat grafting is advised. Additionally a steady weight will help avoid fluctuations after such procedures.
