Is this milia? Can I treat it with microdermabrasion? (Photo)

Great question. From the pictures and your description I would seek out a dermatologist. You may be using something that is causing an allergic reaction. Typically milia or comedones are not itchy. This usually indicates a reaction. I would suggest stopping all topical products and continue with a gentle cleanser, moisturizer only until you can see a derm.

You may want to seek advice from a board certified dermatologist in your area. While facial treatments will help (professional deep facials, microdermabrasion, Hydrafacial and micro-needling), and a suitable skincare regimen, including a retinoid. Thanks for your question and photos.  These lesions don't look like traditional milia, but seem more like closed comedones or a version of comedonal acne.  The retinoids can help improve these, but since you have very dry skin, you will want to get started very slowly with them, especially living in a high desert dry climate.  Microdermabrasion might be helpful and you will want to do a series of them, but also ask about microneedling.  Your dermatologist will be able to evaluate your skin more closely and will likely have some good suggestions for you, so please see an expert, and stay safe!

It's hard to tell from the photos if it's milia.  It looks more like closed comedones to me.  An exfoliating facial with extractions can definitely clear this up.  I would also recommend using a retinol at night.
