Uneven abdomen and hard scar tissue six years after liposuction—what can smooth it? (Photos)

I had abdominal liposuction in 2020. Since then I have been through cancer treatment, major stress and weight gain, so I know some of my current fullness is related to that. However, even when I was lighter and soon after lipo, my stomach was never completely smooth. I have persistent dents, ridges, rippling and several firm scar-like areas beneath the skin. One harder area on my side creates a noticeable bulge or shadow, and I also have fullness around my belly button that has always bothered me. When I stretch or lean back, the skin looks dimpled and uneven, almost like cellulite, but it feels more like post-lipo scar tissue or tethering. I also did not keep up with post-op massages or wear my compression garment as consistently as I should have. My overall goal is to lose weight, have a flatter stomach and more of an hourglass shape, and feel comfortable wearing tops that show my midsection instead of hiding it because I am embarrassed by the lumpiness. I would prefer not to repeat full abdominal liposuction unless that is truly the best one-time correction. Does this look more like fibrosis/scar tethering, uneven remaining fat or a combination? Would targeted scar release, subcision, revision lipo, fat grafting or another approach make sense? Are nonsurgical treatments likely to help this far out, or would surgical correction be more realistic? Should I lose weight first before having it corrected?
