I'm a 31 year old woman who's had a flat midface my whole life. Having little to no definition there really makes my whole face look fatter and rounder in photos. Is it possible to create some natural contour on my cheekbones with Juvederm?

Thank you for sharing your photo and your concerns. Based on what you've described, it sounds like you've always had a flatter midface rather than experiencing age-related volume loss. That's an important distinction because the goal isn't to restore lost volume—it's to enhance your natural facial structure.

From your photo, you do appear to be a reasonable candidate for cheek augmentation with hyaluronic acid filler, such as Juvederm. When placed strategically along the cheekbone and upper midface, filler can create more definition, improve facial contours, and make the face appear less round without looking overfilled. The key is using a conservative approach that enhances your bone structure rather than simply adding volume.

One thing I always emphasize is that more filler does not necessarily produce a better result. In younger patients with naturally fuller faces, excessive filler can actually make the cheeks look wider or heavier. The most attractive outcomes usually come from small amounts placed deeply over the cheekbone to create lift and contour while preserving natural facial movement.

It's also worth remembering that photos—especially those taken with a phone—can flatten facial features and make the midface appear less defined than it does in person. Because of this, treatment planning should be based on an in-person assessment of your facial proportions rather than a single image alone.

Depending on your anatomy, some patients also benefit from subtle enhancement of nearby areas, such as the chin or jawline, to improve overall facial balance. Sometimes improving the proportions of the entire face creates more noticeable contour than focusing only on the cheeks.

Overall, I do think it is possible to achieve a natural-looking enhancement of your cheekbones with Juvederm or another appropriately selected hyaluronic acid filler. The short answer to your question is that there are a number of robust HA volumizing fillers that are routinely used to nonsurgically contour a more feminine, heart-shaped, oval face. That being said, this is best accomplished with HA fillers specifically designed for providing volume. Plain Juvederm or simple Restylane would not be ideal for this purpose of volume replacement and generally would not provide a durable response. You would be wise to make sure that you are consulting with an experienced, board certified cosmetic dermatologist and not just any venue selling treatments of ordinary fillers by the syringe. Your flat cheeks have been inherited from your father‘s side of the family. I would not recommend using fillers, especially in the lower eyelid area since they will create more problems and be extremely expensive in the long run. You will need a large amount of filler to create beautiful cheeks, and you will have to repeat the process every 6 to 12 months in order to maintain the volume desired. My recommendation to you is use your own fat which in the right hands can give you permanent and natural results.
