I want a bilateral lifting and tightening of face and neck
Answer: Thermage vs buccal fat removal Thank you for including pictures with your question. Buccal fat removal is an option, but I recommend you schedule a consultation with a board certified facial plastic surgeon and then you can educate yourself regarding the risks and benefits of the procedure. Thermage is an option and the monopolar current will target the fat to slim your face, but the results are more subtle than surgery. Again, I would recommend you make an appointment and have an examination to go over your options. Best of luck! - Elbert T. Cheng, MD

Answer: Non surgical lifting of the face and neck Thank you for the photos. In my opinion Buccal fat pad removal will make your face thinner but will not lift your cheeks or neck area. I also notice mild asymmetry in the lower face area and am afraid that removing the fat pads may accentuate this irregularity. Fortunately you have many non-surgical options to remove the fatty deposits and tighten the skin. The top 2 devices that we offer for patients in your age group are Thermage CPT and Ultherapy with deep see hand piece. These 2 modalities (Radiofrequency and Ultrasound) skip the top layers of the skin and penetrate 2 to 4mm deep to tighten the skin and the underlying structures. In my experience neither modality melts fat. The skin under the chin may have a little extra fat. This can easily be addressed with injections of Kybella. I hope this was helpful

Answer: Skin Tightening At 36 years old I would definitely urge you not to have any surgery. It is difficult to tell from the photos if there is any submental fat in the area. If there is then a combination of Thermage and Kybella would be best to tighten the skin and remove any fat permanently in the area. I would also do Botox in the neck to tighten the bands there as well. Please consult an expert in Thermage and Kybella for the best cosmetic results. Best, Dr. Green

Answer: Chin laxity Neither procedure would be appropriate. Depending on the degree of laxity, liposuction of the submental region might be possible. Finally, if you need a limited neck lift, then that would be best.
