Q: Is it possible to remove the dark edges of the labia minora in this case? A: Yes, it can be done; however, careful consideration of how to do this operation must be given. In this case, is a little tricky! Curvilinear labioreduction or trimming (linear - straightforward amputation or S-amputation) will not work because there is not enough lighter color under the dark tissues. In addition, the left labia minora presents irregularities with a tiny amount of tissue for reduction. Therefore, suggested procedures will not provide a good surgical outcome in these situations and may resulted in sever complications due to over-resection. Furthermore, in this case, the labia minora size is contraindicated, and doing so, will inadvertently lead to labial over-resection with all potential severe complications (superficial dyspareunia – pain during sexual vaginal intercourse, neuropathy – pain at the site of incision or neuralgia – pain in remote areas from the surgical site, etc.). Solutions:1. Do not do anything, 2. The clitoral prepuce (the clitoral hood or clitoral foreskin) transposition can be used to cover the resected edges of the labia minora; however, the labial length is longer than the clitoral hood, so the lower part of the labia minora and the fourchette will remain dark or reddish when they will be removed, 3. The labia minora tissue is expendable. Currently, my unfinished clinical research shows promising results, and in this case, such an approach can be beneficial. First, it will create extra tissue to remove the dark tissue and create a natural, rounded shape of the labia minora—second, will not cause over-resection. With regards, Prof. Dr. Adam OstrzenskiUSA
