Resection of the hyperactive depressor septi muscle can be done from either an intraoral or an intranasal approach. Release of this muscle will stop the dynamic tip motion downward.
When the nasal tip droops with animation and smiling, it is due to an active depressor septi ligament which pulls the tip down when the smile muscles contract. Releasing of the depressor septi ligament can be performed as a stand alone procedure or as part of a full rhinoplasty procedure.
There are several options for a drooping nasal tip. Botox can often be an effective 3-4 months solution for what is referred to as a dynamic tip. The muscle that causes the droop can also be divided or partially removed with a short surgical procedure. This can be performed with a more extensive rhinoplasty or as a stand-along procedure. I often divide this muscle through an incision inside the mouth to release it and fix the problem of the tip drooping when you smile or talk.
