When the nasal tip droops with animation and smiling, it is due to an active depressor septi ligament which pulls the tip down when the smile muscles contract. Releasing of the depressor septi ligament can be performed as a stand alone procedure or as part of a full rhinoplasty procedure.
Hi Yael,This is a pretty common problem that you describe, but it is more obvious in some than others. It is due to hyperactivity of a muscle that attaches your nose to your upper lip. I often divide this muscle through an incision inside the mouth to release it and fix the problem of the tip drooping when you smile or talk.
