Hello, I always disliked my nostrils, but I know if I get them fixed my nose will look long. I like my profile picture and want to keep it the same, I just want to make my nostrils and tip smaller. I am looking for better look without dramatic change. Do I go with tip plasty and alarplasty or rhinoplasty? Thank you.

You can take Accutane as soon as one month after surgery if the incisions are healed. In fact the Accutane might help improve the overall appearance by reducing thickness related to the sebaceous glands.

In the photo the "bumps" or fullness along your alar base incisions appears to be due to tension along the incision line. Depending on how long ago your surgery was, this may relax and improve with further healing. Without a static photo (not smiling) to compare with, it's hard to rule out...
Skin is elastic and stretches. Especially when smiling, the skin along the nose and ala moves. A 2 mm nasal sill incision will create less than 3-4 mm of width reduction.
