Do I need Alarplasty and Tiplasty or rhinoplasty?

Alarplasty alone can reduce the width and/or flare of the nostrils and ala. It does not change the size or shape of the nasal tip. Surgical rhinoplasty is the safest and most effective way to create permament changes to the size and shape of the nasal tip.

It is very difficult to assess without photos.
However, if width is the issue then Alarplasty alone should suffice. If the tip is bulbous then do both Alarplasty and Tip-plasty. These two will give refinement without a drastic change and should maintain the normal look you have at present. Do more research so you understand the terms better.

"I know if I get them fixed my nose will look long." I've never heard of that happening with nostril work.

And whether work on your tip would make you happier depends 100% on what the tip looks like now.

And one more point: there is no difference between a "tip plasty" and a "rhinoplasty." A "tip plasty", such as it is, is just a rhinoplasty, where most of the work, and perhaps all of the hoped-for changes, are in the tip.
