What is causing this irregularity after SMAS Facelift? (Photo)

Some questions I would like to ask are: 1. Do you think these irregularities was caused from Cheekbone Reduction, and just didn't become apparent untill after facelift? 2. Are the ridges/bulges made up from fat, scar tissue, ligaments, scar tissue contracture, scar hyperplasia, etc? 3. Which surgeries/method could help correct these impressions? Deep plane facelift revision, Smas facelift revision, facial liposuction, etc. (More description in the photos)
