The first pictures are my breasts and the others are my wish pictures. I would like my implants to be as big as my body allows. I know I would have to be assessed in person, but I would like more opinions before I start going to consultations. I'm looking for saline, round, smooth, high profile, sub muscular and a transaxillary incision. Thank you doctors.

Thank you for your question. It is much better to accurately discuss implant volume and projection with an in office exam and detailed measurements. In order to make an accurate size recommendation, I would need to assess your chest wall and breast mound measurements/characteristics. I use these measurements to help me during the sizing process to guide you to a safe result for your chest wall and just as important, one that meets your end desires.The next step is to try on this range of implants in the office with your doctor. The key to this success is showing your surgeon the body proportion you desire with a bra sizer and allowing your surgeon to guide you to the right implant. It will be much easier to communicate in implant cc's than cup size when determining the appropriate implant for you during the sizing process. In addition, review of photos and use of rice bags can be very helpful to the patient and surgeon in achieving a safe and happy result. You have a good breast shape and will get a  nice result with a breast augmentation.  An experienced plastic surgeon should be able to help you make a decision about implant size.  You could use very large implants up to 800 cc but one should keep in mind that time, weight  and gravity are not on your side.  Large implants will accelerate the aging process of the breasts.

It would be helpful to know the width of your breasts before make a recommendation. Most surgeons try to avoid using implants wider than the patient's own breasts.  At these volumes you would be looking at about 12.5 cm. That will be a rather large augmentation on someone of your frame and breast size.  Hopefully you have had the chance to try on sizers as part of the decision making process. With implants that large, I would carefully review the pros and cons of silicone gel implants. You are lucky that you have a good shape to the breasts as a starting point so that will help you achieve your goals!  It sounds like you have done your research and know what you want so good for you. I think you have also chosen some reasonable pictures for your goal look which is helpful.  I think all of your choices are quite reasonable for you.  I tend to prefer the silicone implants because they seem to be a little softer and offer a more natural feeling breast in my opinion but saline is a reasonable choice if you don't mind them feeling a little more firm.  Your size range may be reasonable but at your consultations with plastic surgeons your breast measurements will be taken and using the base width of the breast we can help determine if in fact that size range is appropriate. I do like having my patients give me a RANGE with implant size because that gives me a little wiggle room in the OR to adjust to what looks best with intraoperative sizing.  I also like a submuscular approach because it seems to have a lower risk of capsular contracture than subglandular.  As for the transaxillary incision--not my favorite because I think it sometimes becomes difficult to really expose and open up the lower inner pole of the breast to really set the implant position appropriately to have good cleavage and shape to the curve of the breast.  I prefer an inframammary incision as it also has been shown to have lower capsular contracture rates and gives you direct and relatively straight forward access to the submuscular pocket and is not near any nerves. Plus for most women with larger than an A cup, it is essentially hidden in the fold of the breast anyway.  When you choose your surgeon, please just make sure you are consulting with board certified plastic surgeons with experience in cosmetic breast surgery. Good luck!

Thank you for the question and pictures. Base on your photographs, you are starting at a great place and should have a very nice outcome with breast augmentation surgery.  Breast lifting is not necessary. Based on your starting point, I think you will have a very nice outcome regardless of the breast implant type utilized; therefore, it will be important for you to learn about the pros/cons associated with saline versus silicone gel breast implants. You may find the attached link describing the differences helpful to you.   Generally speaking, the best online advice I can give to ladies who are considering  breast augmentation surgery ( regarding breast implant size/profile selection) is:
 1. Concentrate on choosing your plastic surgeon carefully.  Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you're looking for. Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your   desired goals  with your plastic surgeon. This communication will be critical in determining which operation and/or breast implant size/type/profile will most likely help achieve your goals.
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C or D cup" etc or " lovely cleavage” means different things to different people and therefore prove unhelpful.
 Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup  size may also be inaccurate.  The use of  in bra sizers and computer imaging may be very helpful during this communication phase.
3.  Once you feel you have communicated your goals clearly,  allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals.  Again, in my practice, this decision is usually made during surgery.  I find that the use of temporary  intraoperative sizers ( with the patient in the upright position)  allows for accurate selection of breast implant size/profile to achieve patient's goals as precisely as possible.

Congratulations on doing your homework as you seem very well informed.  Based on your description of your size you are a very petite small framed woman.  Breast implants 500 cc and over are quite large and do carry risks increase capsular contraction.Personally and a petite small woman I typically do not go over 400 to most 450 cc.  NextHowever your size is your choice.  Please consult a board certified plastic surgeon and make sure the Dr. has of breast implant sizing system so that you can see how you will look with various sizes.
