You are correct, explant and lift is your best option.  The technique I recommend is The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained, if not injured during your previous surgery.  At the same time or later, fat transfers can be performed if additional volume is desired.Best Wishes,Gary Horndeski, M.D.

Thank you for sharing your question and photographs and I am sorry to hear of your difficulties following revision great surgery.  The best treatment can only be determined based on your ultimate breast goals.  In addition to your mentioned issues you also have excess breast skin that would benefit from a lift procedure.  The best options would include either implant removal and a lift, or conversion of the implant to above your chest muscle and a lift.  I would recommend seeing a series of board certified plastic surgeons who can perform an in-person evaluation of your implants and breast tissues to offer the best recommendations.

Your best option may be to place the implants above the muscle to eliminate the window shade problem.

For your best result, a lift is needed. If you are bothered by being able to feel the implant, then you should explant them. You are thin so you will always be able to feel them. If you are bothered by the animation deformity, change the implants to a shaped implant and they can be placed in the prepectoral space and your muscle can be returned to the chest wall. A combination approach would be best but ultimately it depends on your goals and what you will be happy with.
