Determining whether you need simply a breast implant versus a breast lift or both can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position.

Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift.

These two surgeries can be done at the same time and this may be optimal as the risks can be minimized. 

My preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift).

Most importantly, it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, it is actually breast tissue that is being brought together to support the breast and hence improves longevity.

In addition, upper pole fullness is achieved with appropriate implant selection but more importantly, submuscular placement, and precise pocket dissection. The submuscular placement allows the muscle to cover the top part of the implant which allows the upper pole to appear fuller. Keeping away from the lateral chest during pocket dissection allows the implant to remain higher and prevents it from falling off to the side.

You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Thank you for your questions and congratulations on the weight loss.  Looking at your pictures I would recommend having a Breast lift done and then a few months later have an implant place.  The reason I would separate the procedures is because on your left side you are significantly droopy and if we place the implant at the same time we may not be able to lift the breast as high as we would like.  By adding the implant later it will give better shape to the breast, add fullness and firmness as well.  I would suggest a silicone gel implant underneath the muscle.   I would also suggest speaking with a Board Certified Plastic Surgeon and being evaluated in person to make sure you are a proper candidate and to answer any questions and concerns.  Best of luck to you. 

Yes, of course. Any good cosmetic surgeon doing lot of mastopexy can help you get good results since you have good volume of breast but they are saggy. So basically what you need is a mastopexy procedure also called as a breast lift so that you achieve a nice contour and shape of your breast which is lifted.

Congratulations on your weight-loss! The best procedure for you is dependent on exactly what your aesthetic goals are. Based on your photo, I think you are a great candidate for a breast lift. A breast lift will address any sagging skin and breast tissue and improve your breast shape. A breast lift will also help create symmetry by lining up the nipples. If you are also looking for an increase in breast volume, a breast lift with implants is an option to consider as well. Be aware that future weight change and/or pregnancy could change your breasts or any surgical results. Your breasts could be enhanced with a breast lift. Thank you for the question and images. Congratulations on the significant amount of weight loss that is a real achievement. In my practice I would offer you to main types of reconstruction. An augmentation mastopexy with implants or a auto-augmentation and mastopexy. The latter utilises the excess skin and subcutaneous fat from your back to augment and add volume to the breast. This can avoid the need for an implant to create upper pole fullness. It obviously needs to be discussed in detail as it involves longer surgery and scars on the back. Based on your photo, it appears that you would benefit from a breast lift surgery possibly with or without implants depending on what you want. hello thank you for your question and provided information as well.. based in your question and the picture you showed you are a good candidate for breast augmentation with implant with T inverted  with incision ., in your case its recommended get a consultation in personal with a plastic surgeon to determinate how many cc do you need to get a very well result.

Thank you for your question and photos. I would advice a breast uplift without an implant if possible. In general, patients post weight loss have thin skin which can not hold up a heavy implant. The uplift alone will improve the shape, the nipple position and can also make the breasts look larger. Best of luck.
