Do not feel guilty about your sagging breasts ,because it has nothing to do with what you do but rather is your genetic makeup which there is not much that you can do on a personal level. The good news is that plastic surgery can help you considerably with your self image and  self confidence. You did not provide information about your breasts size which is important because that will determined if you need just  breasts lift or breasts reduction with a lift. That can be easily solved on your consultation with a board certified plastic surgeon. Make sure that the surgeon operate in an accredited surgery center and check the before and after pictures, so you feel comfortable with the quality of surgical outcome. Depending on the fullness of your breasts, you may choose to havea breast augmentation with the lift, or a breast reduction.
