I had an excellent experience at Form Medical Spa. Mitch did an amazing job with my HydraFacial. To start he was so welcoming and just very friendly. I've never had any kind of facial before, but he walked me through the steps and was really knowledgeable about the whole process. I left with my skin feeling 1000x better than before.

Neck and Lower Facelift at 70 Years Old

Dr P. Daniel Ward is the very best surgeon. He is very knowledgeable, compassionate devoted to his patients. I always get the Rolls Royce treatment from Dr. Ward and his wonderful staff at Form Spa!
That's why he received Top Doctor Award by his colleagues. If your looking for an excellent doctor , Dr. Ward is the one. I have recommended my two sisters, one living in San Diego. My daughter and grand-

Dr Ward, Traci, Sean & the staff at Form Spa were amazing. They made me feel very comfortable & were very accommodating.
Very pleased with my results.
I was extremely nervous about my surgery & they literally calmed all my fears & walked me through each step of the surgery, expectations, post surgery care & follow up.

30 Year Old Female, Always Had Chronic Sinus Infections, and Didn't Like my Nose

I have had chronic sinus infections for years and years. I also, never liked my nose. I was looking for a doctor that specialized in facial plastic surgery and that I trusted to perform Septoplasty and Rhinoplasty. I went to a consultation with Dr. Daniel Ward at Form Med Spa. Rhinoplasty

My nose was a very big insecurity so I got a rhinoplasty a couple months ago and the results are amazing! Looks very natural which is what I wanted. Form Med Spa is the place to go, everyone is so friendly and you definitely leave happy!

Botox isn't so scary after all - Salt Lake City, UT

Let me start off by saying that I am 25 years old, currently living in the Cottonwood Heights area, and I NEVER write reviews. However, I believe that great work deserves great recognition. I recently visited Form Medical Spa, located in the Old Mill office park, and had an amazing experience. My first thoughts from walking into the Form Medical Spa office was that Dr. Wards new location was extremely clean, well designed, and the beautiful decor gave it a warm modern feeling.

Dysport at FORM Med Spa

I'm a 30 year old female who has been interested in Botox for several years with my increasing forehead wrinkles. I just haven't taken the time to go in mostly do to fear of cost and the process. Three weeks ago I had my first treatment with Dr. Ward at FORM and am so so thrilled with the results! No Refunds

Make sure you know they do NOT give refunds-even when they are in error. I honestly had the best experience with my first visits, but it all fell apart in the last month. I went for a laser hair removal treatment on my legs. The tech could not get the laser and cooling air to work properly. I expressed that my legs were burning and she told me it was normal. I told the front desk before leaving that they were on "fire" and told to see how they felt when I got home. I called
