56 yo mother of one from Santa Fe, New Mexico located the office of Board Certified, Fellowship-Trained Urogynecologist and Master Cosmetic Surgeon Dr. Oscar Aguirre of Denver, Colorado through an on-line Real Self search. She is a woman with a long history of undesired cosmetic appearance of her vulva as well as difficulties with achieving orgasm. She is interested in vaginal rejuvenation options to improve the appearance of her vulva. She states she is looking to reduce her hood and make her vagina overall tighter. She is also looking to improve sex. A virtual consultation was arranged and photos she had submitted to Dr. Aguirre, where reviewed with her. During this initial consultation with Dr. Aguirre she relayed wanting to improve her sexual relations and make orgasms easier to achieve. Review of photos revealed clitoromegaly due to testosterone. The patient was aware of this being the cause. 40 yo mother of three from Denver, Colorado self-referred to renowned Cosmetic Surgeon and Board Certified, Fellowship-Trained Urogynecologist Dr. Oscar Aguirre of Parker, Colorado to address vaginal looseness. She has a lifelong history of undesired cosmetic appearance of her vulva due to large and dark labia minora. She has also been bothered by a 20-year history of vaginal looseness following her vaginal deliveries. She has been married for many years and both her and her husband noticed decreased sexual pleasure and sexual difficulties. Dr. Aguirre performed a through pelvic examination which revealed mild urethral hypermobility and posterior compartment relaxation. Distally there are torn pelvic floor muscles contributing to her laxity and her husband falling out. There is bilateral labial hypertrophy with hyperpigmentation and redundant lateral prepuce with ptosis. Her G-Sot region is non-sensitive. After discussing all surgical and non-surgical options she decided to move forward with in-office procedures as recommended. She feels that her vaginal looseness is impacting her sexual relations. She complains of decreased sensation despite her partner having a very large penis. She wishes her sexual experience, pleasure, sensation, and orgasms were better. She also reports a 13 year history of stress urinary incontinence. On further questioning, she is bothered by the aesthetic appearance of her vulva. She feels her labia and hood are too large and her labia majora and her inner thighs are too dark. Dr. Aguirre performed a through pelvic examination which revealed hyperpigmentation of her labia majora and inner thighs, bilateral labial hypertrophy with hyperpigmentation as well as redundant lateral prepuce. Inspection at the introitus reveals right periurethral tear. There is significant vaginal dryness for her age. She performed a weak pelvic floor contraction proximally, and distally there are torn pelvic floor muscles. She did experience pain with sex after her first delivery, but this resolved following her second vaginal delivery. Her labia bother her with horseback riding, and also with intercourse.
