My patient had undergone a 160lb weight loss after bariatric surgery. He had heavy eyelids, bags under the eyes, neck banding and jowling, with severe volume loss in his face. His under eye area was exacerbated by both the under eye fat bags as well as the volume loss in his cheeks. He underwent a Face and Necklift, Upper and Lower Blepharoplasty, and&nbsp;a Fat Transfer to return his youthful cheek contours, soften the under eye hollows and the nasolabial and marionette lines. This beautiful patient came in complaining that her lower eyes looked tired and she felt she had bags under her eyes. eyebags are often a hereditary trait and can be seen in patients as young as 20s and 30s. This patient was looking for a long term more permanent solution that doesnt require ongoing treatments. She opted for a lower blepharoplasty with fat transfer and laser resurfacing. When this procedure is performed at a young age often it can be done without any skin incisions on the face. The fat transfer allows subtle contouring to the cheek, face and tear trough to avoid hollowing.

My 38 year old patient was complaining of appearing tired and her eye make up was no longer able to cover up the bags and darkness under her eyes. I suggested she undergo a transconjunctival approach to the lower eyelids, to remove the excess fat pockets which was worse on the right, then I also suggested a facial fat transfer to the upper cheeks to improve the darkness and hollows and I suggest CO2 laser skin resurfacing around the eyes to improve the fine lines, wrinkles, tighten skin and raise the brow and upper eyelid skin slightly. Here she is 4 months post op. Her under eye bags are gone, the skin is soft and smooth in appearance and the shadows are&nbsp;removed from the fat transfer. Even her upper eyelids appear more open and brows appear elevated from the CO2 laser skin resurfacing around the eyes and brow area. All the procedures were performed under local anesthesia and took about 2 hours to complete.
