The lack of mid-face support is seen in the pre-surgical photo.  It was determined that the lower jaw was in proper position (the gender and airway space on the X-ray was also considered). Therefore, the upper jaw was advanced forward instead along with genioplasty.  A youthful face is accomplished when there is good structural support.
