After years of dealing with breathing and sleeping issues, I decided it was time to seek professional help. I waited 3 years to have my surgery with Dr. Bell, and I’m really happy that I did. Dr. Bell fully addressed my concerns that a lot of other surgeons would not. For reference I came to inquire about jaw surgery because of health issues and recession caused by prior orthodontic treatment. When I was younger I underwent camouflage orthodontics, and it pushed my face back into my head. As a result I had poor breathing, poor posture, pain, and self confidence issues due to lacking in facial structure. Dr. Bell is one of the few surgeons out there, in my experience, who works with patients to correct issues specifically caused by extractions/ retraction camouflage orthodontic treatment. Dr. Bell listened to, and validated all of my concerns when most other surgeons were dismissive simply because my pre surgery bite was okay. Not only was Dr. Bell focused on my health concerns, he also worked hard to give me the aesthetic result that I wanted. He is capable of providing patience with a lot of projection as well as stability. I have had no pain after surgery. I’m very happy with my results. I needed jaw surgery and was referred to Dr. Bell by my periodontist. My periodontist did not trust his patients with any other surgeon for jaw surgery except Dr. Bell. Dr. Bell is kind, meticulous, and extremely knowledgeable in his specialty. He has been doing jaw surgeries for 20+ years. He previously worked for Kaiser but now has his own practice. More than half of Dr. Bell’s cases are patients who come to him for a revision due to a previous jaw surgery gone wrong. Dr. Bell individualized my surgical plan, revising it over several months until it was the best fit for me. Safety is always Dr. Bells main concern for his patients. This review is for a consultation with Dr. Bell; Pros:There was plenty of time allocated to the visit, 1.5 hours. The office staff was courteous, the doctor seemed to want to help me with my case. He understood the frustration of trying to find the right help in the medical world. He spent a good amount of time reviewing my scans with me. He sends an overwhelming amount of information prior to the visit, some of which I found helpful. I walked away with reassurance that I was going down the right path. Cons:Dr. Bell spent little time building rapport, and the consultation started abruptly, followed by what I felt was a disorganized appointment. He was unprofessional at times throughout the consultation and was interrogative concerning personal relationships, my job, and condescending towards previous doctors I have established relationships with, some of which he had little to no information on their background or experience in their respective fields. I was also interrupted while trying to answer questions the doctor had. One of the documents I received was “8 Questions to ask your surgeon about orthognathic surgery”, which I added some to my personal list of questions I wanted to ask during my consultation. While some of these were answered during our time, the meeting ended abruptly, and I had no opportunity to ask any questions I had on my list. Dr. Bell spent approximately 10-15 minutes screensharing a thread on Reddit, where he had an exchange with another poster, and read line for line his back and forth with the individual. The content had no relevance to my case but used this time to make the point that there was a lot of misinformation on Reddit, something that we both agreed on.Lastly, while Dr. Bell was screensharing, approximately 3 times he scrolled through a folder showing the names of patients, which concerned me about the privacy of my visit and information.I gave 2 stars because of the pro’s listed, but I cannot recommend this doctor for consultations.
I've so far only done a consult with him, but, if you're reading reviews, you're probably not booking surgery tomorrow: you're deciding your shortlist for who to do a consult with. So, for that, I can definitively say that Dr. Bell should be on it. Regardless of if you end up going with him or not, you'll learn a ton and won't regret it. He puts -a lot- of time and thought into his consults, really thinking through the details of your particular case and giving quality explanations of his thought processes. Jaw surgery is a huge decision. Unfortunately, it's not (yet) like buying a car where you can go onto manufacturer websites and learn about everything you need to know to make a decision. Very few surgeons want to spend the time individually walking you through what you need to know; Dr. Bell is one of them.
Your concern about your lower jaw being recessed vs. a deep labiomental fold, together with your concern about a lack of projection of your midface are intimately related. Indeed, you are describing a facial growth pattern that can be optimized with orthognathic surgery. The facial photos that you have provided give some clues to solving this geometric puzzle. However, to really understand the structure of your face and fulfill your cosmetic wishes, we would need a CT scan and perform a much more thorough evaluation. For example, in a profile view, whether you have your head rotated in a clockwise versus counterclockwise manner significantly affects the forward projection of your chin. The data from your CT scan (referenced above), together with a thorough physical examination, can then be merged with a three-dimensional photo image of your face using a 3dMD imaging system to design your face exactly how you want it to be. I recommend that all patients review with their surgeons their surgical treatment plan based on what is referred to as a 3D VTO (three-dimensional visual treatment objective). In other words, a document that would be sent to you so that you can consider over a couple of days whether you are satisfied with the plan, i.e. design for your face, that you have collaborated to create with your surgeon.
Hi. The appearance of your face is consistent with maxillary anterior vertical hypoplasia (meaning that the front part of the upper jaw is very short in the vertical dimension), otherwise known as "short face syndrome". With this kind of facial growth pattern, people typically do not show any upper teeth in a relaxed state, and display very little teeth when they smile. Quite often, but not in invariably, the upper jaw has also not grown sufficiently far forward. Because of this, the nose may look disproportionally large when in fact, the nose is a normal size. It simply appears this way because there is a relative imbalance. All of this is fixable in a highly cosmetically pleasing way, that is simultaneously optimally functional, and in a manner that can provide long-term stability. This can be performed with the art and science of orthognathic surgery.
Your bite cannot be corrected with braces alone unless you are willing to have teeth extracted from your lower jaw and have your orthodontist create what is known as severe compensations in the positions of your teeth. If you are willing to accept these severe compromises, this may be your best choice. However, while your teeth may fit a bit better together, the overall appearance of your face will not change significantly with braces alone. If you do not address this misalignment with surgery now, you may not experience any problems whatsoever. However, most states of biologic abnormalities have what is known as a "natural history" that includes a slow, progressive downhill course. I do not know what type of problems, if any, you are having at present. In some people with a facial structure such as yourself, the consequences of not having jaw surgery may include: ⦁ Abnormal tooth wear. Wear can be significant enough in some cases to require the placement of crowns.⦁ Abnormal mechanical forces resulting in teeth loosening and premature tooth loss.⦁ TMJ problems include muscular pain, arthritis, chronic pain, and progressive joint deterioration.⦁ Continued or worsened difficulty with chewing and speech.⦁ Psychological consequences including low self-esteem due to societal conditioning and the value placed on physical attractiveness, impairments related to social anxiety, and, tragically, the increased risk of ostracism and social isolation, bullying, ridicule, and other forms of abusive behavior. Most of us know these events are commonplace among adolescents and teens. The long-term consequences of these psychological traumas, especially in the developing brain, often lead to lifelong impairment and become "frozen in the psyche" over time. Sadly, they are usually left untreated.⦁ Even if the above consequences do not occur, right or wrong, the reality is that due to the value that our society tends to place being on being young, handsome, or pretty, loss of romantic opportunities and career opportunities are commonplace. The attractiveness of one's face has been described by some as a form of "social currency".I wish you well.
The symptoms that you are describing are extremely common and are frequently associated with one another. The answer to your question about the pros and cons of having a mandibular advancement can only be answered by the surgeon who is performing the surgery, based on your surgeon's personal outcome statistics. There are many different types of complications, and it is important for you to know that there is a vast difference between you, the patient, having a 1 to 2% chance of a major complication such as having permanent numbness or needing revision surgery, and a 5 to 10% chance of having a minor complication such as a low-grade infection that clears up within a week after surgery. The scientific literature is full of studies about every conceivable complication associated with orthognathic surgery that could possibly be imagined. However, what is most important for you to know is how many procedures your surgeon has performed. It doesn't matter if your surgeon has been performing mandibular advancements for 20 years, because if they are only performing a few number of these procedures per year, they will not have much experience, they will not have made mistakes (and, hopefully, learned from their mistakes) and they will not have a vast body of personal statistics that they can quote from regarding their results. Ultimately, this is the information that you, as a healthcare consumer, will want to know. You may also want to see their before and after results, read reviews about your surgeon, and speak with other people that your surgeon has operated on of the same age, same gender, and the same facial structure as yourself to have a better idea of what you can expect. If your surgeon is unwilling or unable to provide you with this information, I would think twice about that surgeon. My recommendation is to choose your surgeon wisely.
Recovery from double-jaw surgery follows a generally predictable biologic time course as follows: Three days after surgery, swelling is at its maximumtwo weeks after surgery, swelling is down to 50% of the maximum six weeks after surgery, swelling is down to about 80 to 90% of its maximumone year after surgery, the swelling should be essentially gone You may assume that your face's shape is due to swelling, but this may not necessarily be true. It may be due to the shape of your facial skeleton. Unfortunately, many surgeons perform jaw surgery without much communication with their patients and only verbally describe what their patients' faces will look like after surgery. Furthermore, different people have different ideas about what looks ideal. Computer-based virtual surgery planning is the standard of care in North America and many other places worldwide. During this planning process, people who are having surgery should be involved in the planning process and provide feedback, express their cosmetic needs to their surgeons, and communicate with their surgeons how they want to look. This is the very basis for what is known as informed consent. In other words, people must consent to what will be done to their faces. In my experience, it is not possible to fully communicate with my patients by using phrases such as "a more prominent chin," a "shorter face," or "a fuller midface." Actual predictive images or computer-based simulations my patients can think about for a few days and approve are necessary. If my patients don't like what we planned TOGETHER, they are given time to let me know so I can change their surgical plan. Furthermore, I can superimpose three-dimensional photographic imaging on their CT scan during the planning process, with the CT scan being the basis for computer-based planning, further enhancing my patients' understanding of what to expect. Simply stated, a picture is worth a thousand words. I recommend consulting with an expert surgeon. The surgeon should obtain a new CT scan of your face and examine the structure of your facial skeleton and the amount of swelling present. I wish you well.
