I spent over a year looking, and I mean REALLY looking for a doctor to perform my rhinoplasty. I had a ugly dorsal hump, wide nose, and droopy tip. I spoke to many of the "best" surgeons Texas had to offer. However, none of them could offer me what I really wanted. Sure, they could perform a rhinoplasty.. but they wouldn't perform it to my comfort. I wanted to have my procedure done strictly under local anesthesia for my own personal reasons. Everyone turned me away, or tried to pressure me into doing this surgery under IV sedation or general anesthesia. Then I met Dr Apostolakis. He didn't turn me away. He didn't pressure me. He made sure I was informed and agreed to do my procedure. I had my procedure, with local anesthesia, AWAKE. My nurse Kim was AMAZING, she held my hand through the worst part which was numbing my nose. After that, it was a breeze. I was able to have a full on conversation with Dr Apostolakis and Kim the entire time. I did not feel a single thing. I had minimal swelling the day after surgery, minimal bruising, there was no pain afterwards at all so I didn't even need Tylenol. The whole process from beginning to end was great. Ali was also amazing, she is the sweetest person I have ever met and was so nice to me throughout the entire process. If I could rate Dr Apostolakis and his staff more than 5 stars I would, I only wish I would have found him sooner. I am one week post op, hump and droop free and I LOVE it.
I searched for years for a Dr in Texas who's work I liked enough to even think about following through with a Rhinoplasty. There's plenty of "Instagram doctors" out there to look at, but none who's work I thought looked natural and consistently good. Thousands of google searches I found Dr.Apostolakis and was impressed with the before and afters on his website. Something to think about here is a good doctor probably doesn't have to advertise on IG.. Anyways, I had a nose that I felt was too big and not proportional to my face. I had a small bump on the profile and it veered to the right. Those things didn't concern me so much, as I just felt it was too projected outwards for my profile. I learned to live with my looks as a whole, but fell in love with Dr.A's work. I was convinced! And I'm so happy I did! I feel so much more feminine and don't worry about how my nose looks on my face. He did a lovely job making it look more straight, less projected, and even. I love my new look and new found freedom of not caring about a feature I had no control of. The healing process was shockingly better than I could ever have hoped. I understand everyone has different pain levels but the medication must have been real good haha. I only took the pain meds about two days(?). I didn't swell or bruise too bad like some photos I had seen on some people's journey. I did take Arnica and drink a ton of green juice hoping it would help. I felt perfectly fine during the healing process. No discomfort, but I did have the sensation of my nose "healing" or feeling different in some way. It's hard to explain. After a week, the bruising was nothing but a sliver under my eyes and the cast came off! I was very pleased! It's now been three months and any sign of swelling that I can tell seems to have disappeared. Oh and during the consultation, before surgery, and the check up Dr.A is very informative. It's so hard to find tear trough implant information since everything is filler-geared so hopefully this will help. I was born with deep hollows that really intensified in my 30's. The tear trough line was actually one of the most defining features of my face. By the time I was 40 no amount of concealer would help. (Fillers were great but I have a high metabolism so my body would eat through it in a few weeks.) I did the math and figured that keeping my tear troughs maintained with filler would cost me at least 3000 per year, when I could have a permanent solution done for a one time roug estimate of 6000. I looked into surgery and found one of the most highly regarded experts in this lived in Austin (Dr. Apostolakis.) I met with him and he gave me confidence that he knew what he was doing - he's pretty direct in a wry way which I appreciated. So, I had the procedure done in May of 2019. Swelling, bruising was as expected, no complications though it took 3 months to really see the long-term results. Here's what I discovered: The Pros: 1. That deep crease that I spackled daily with concealer was -poof- gone like magic. 2. Cheekbones: This was not expected but I have more definition to the mid-plane of my face which is a nice side effect. 3. Dark circles are greatly minimized. 4. Beside the surgery, I should also point out that the Dr.is a filler wizard. He did my filler/botox combo on the glabellar frown line between my eyebrows which has lasted for over a year and is still going strong. Nobody has ever done this on me correctly. Seriously, most doctor's don't inject but he's brilliant with this. The Cons: (deep breath) 1. Nerve pain: 1 year later parts of my face near my sinuses are still numb or painful in other areas. 2. Swelling is still evident a year later. Worse on some days. I don't know if this is a mild infection or if my body is just rejecting a foreign body. 2. This is the scary part: My vision has changed. I have always been far sighted but almost instantly my close up vision became blurry. I mentioned this to the Dr but he said it was unrelated. However, apparently injections/surgery can irritate or damage the ocular nerve. Jesus - How am I even supposed to know if my ocular nerve is damaged? Is there even a procedure to test for this? Who do you talk to? The dearth of information regarding this procedure makes it very hard to get straight info. (Especially when most Doctors will simply chastise you for getting the procedure in the first place.) Look, I have no issues with Dr. Apostolakis - he's a great surgeon and the procedure did what it promised to do. My concern is that my particular anatomy isn't handling this procedure well and I don't know how to proceed. Did I just blow 6000? If I have to have the implants removed will my vision even improve? Will the swelling ever go down? Will my hollows be even worse because of all the manipulation to the soft tissue?
This is long overdue review for this caring doctor. I had a bad accident & was in denial that my nose was broken due to a fall. It wasn't until 2 weeks later I realized my right nostril breathing was a bit shallowed. I interviewed a few local DFW doctors & determined that indeed my nose was broken. I had to have it fixed immediately & wanted to find someone who had operated on Asian noses specifically. I even contemplated of going to Cali but the follow up would have been a nightmare. Even though I'm in Dallas, I went ahead & booked for an appt that was 3.5 hours away. Dr. Apostolakis was very honest with his opinions & told me his method of making it straight again. This was a big deal for me because I don't like needles let alone going under to fix my nose and septum. His nurses took great care of me & I don't remember being in any pain. The recovery was a long time indeed and sleeping upright was the only thing that was uncomfortable. The first few months I was concerned because my nose didn't seem like it was straight and I kept bugging him each time at follow up it was still crooked. But Dr. A was very patient in explaining to me that it would take at least 12 months to fully recover due to it being swollen. Today, I'm at 10 months and I feel the swelling has gone down drastically and no one can tell that I even had an accident. I still need to see him for my follow up but I am very happy with the results and HIGHLY RECOMMEND this wonderful doctor. He also did a small bleph for me while other doctors were hesitant to do so. I can attest that he knows his craft and has a great attention to detail.
Hi everyone! 2 months ago, I had a rhinoplasty by Dr. A and I just wanted to come on here and share my experience. This is is something I've wanted to do for a long time and finally got the courage to do earlier this year. I chose Dr. A because he came so highly recommended and had beautiful before and after photos, but also because he's an ENT (ear, nose, throat doctor) as well as a plastic surgeon. This surgery was life-changing for me. If you are interested in having a rhinoplasty, I would highly recommend him and his staff--they made me feel safe and took the time to answer all my questions. I want to show some before and after photos I took below because the change is actually really drastic but also looks so natural. This photo only two months post-surgery. Thank you Dr.A!! I had tear through implants placed under the eye November 5 of 2018. My eyes were very sunken and looked really deformed. Dr.A did an amazing job at bringing them back to life. I also had laser resurfacing around both eyes on February 14, 2019. This procedure gave me a 75% reduction of under eye wrinkles. Dr.A and his staff are totally amazing. Dr. A tells you upfront if any type of procedure you’re looking for will help or not. He’s straight to the point. He will not perform any surgery that is not needed. Dr. Apostolakis was very realistic; my tear troughs were the deepest he'd ever seen; everyone in my family has these troughs so its genetic just having a hollow under eye. He said point blank w/o a custom implant, the deepest implant would solve maybe 80-90% of my troughs but they'd still remain. This is exactly what I got but I am SO happy. The difference is noticeable but also I still look like a better version of myself. I researched for 4+ yrs on Dr. Apostolakis and I'm so glad I decided on him; he seemed to be the best and he IS the best; the tear trough implant expert. I would also like to thank him for talking me out getting additional unnecessary implants; the tear trough implant has brought out my cheekbones, has made even my nose look better; it was the only thing I needed. I have excluded the photos that included the swelling and healing I dealt with; depending on who you are you might swell alot or a little. I swelled up alot and lymphatic massages really helped to get the swelling down faster but my body naturally swells and it takes forever to get back down to normal.
I always had a slight jaw and weak chin, and my nose is prominent too, so having a weaker chin really makes the nose stand out. Though i have wrestled with the idea of a rhinoplasty for years, I recently became motivated to take measures to just counterbalance my nose rather than change it. Dr. Apostolakis and his nurses and aides were all very competent and professional, even though I'm a bit neurotic about doctor visits in general. The doctor told me that with chin implants, he offers to use local anesthesia so the patient can be awake to see what different implant sizes look like on them and choose their favorite, and that the incision can be made either inside the mouth or under the chin where it will leave a small scar. Because i also asked for lipo, we did full anesthesia and an under chin incision. I had the procedure done yesterday and was sent home with a tight bandage around my head, constricting my mouth and ears, which wasn't a good time. Today they cut the bandage off and next week I'll go back to get stitches out, and I'll be able to remove the tape in a couple days. I'll post the pics of before, during and after once i get stitches out, and I'll try and post again later on down the road once things have settled. Updated on 19 May 2019: Tomorrow I'll be able to remove the tape that is holding things in place while the implant firms up its position in my tissue. The bruising isn't really noticeable to me except for the little raspberry over my "suprasternal notch" (the little dip at the very base of your neck beneath your chin). The skin is a little grey beneath my jawbones too, but it's quite faint, considering. The pain isn't much now. I stopped taking hydrocodone for it yesterday and switched to Motrin. It's just sore under the skin of my neck, and I suspect it will be for a while. My chin and mouth, though, are totally numb. I can't feel my chin at all and I'm just now starting to regain feeling in my lower lip, which should make eating in public less embarrassing. I'm trying to avoid caffeine and wine, but I haven't been able to stop smoking entirely. I know this is likely to slow the healing, but as far as I can tell, everything is coming right along. I'm eating bananas every day to help myself retain moisture (potassium and magnesium), and I'm guzzling down water all day long and trying my best to get good sleep, even though it's difficult for me to sleep on my back. From what I've read, I should be seeing the final results of the lipo within about six to eight weeks, with the skin all firmed up and settled and bruise-free. The Jay Leno-esque chin should chill out within a few days to a few weeks, but I'm reading that it won't quite look totally settled for a few months. That kind of depends on the person and their habits though, so we'll see. Updated on 8 Aug 2019: This was a really good decision. If you need anything like this done in the Austin, TX area, Dr. Apostolakis can be counted on to be honest about the right procedures for you and to provide high-quality results. Updated on 30 Aug 2020: Glad I saw Dr. Apostolakis do this! It looks perfectly natural now, one year and three months later. I think the lipo has mostly held up, even though I don't do any more or less exercise and dieting than before, and the chin has the desired effect. It 'evens up' my face a lot, making my prominent nose look more at home with my other features. I feel so much better about the way I look, even though it was a fairly small change. I let the Dr. choose the size that he thought was best. He chose the right one for its width, and then he shaved it down for the right projection. Dr. Apo knows his stuff.
Hi everyone! I wanted to share some of my rhinoplasty journey. I just had an open rhinoplasty a little over a week ago (Thursday, February 28, 2019). The main reason I wanted a nose job was because I had a pretty big dorsal hump that always bothered me. After consulting with some surgeons we agreed it would be best to also do a minor tip reduction as well. I ended up choosing Dr. Apostolakis as my surgeon and he definitely exceeded my expectations to say the least. Dr. Apostolakis is a very talented plastic surgeon and is obviously very knowledgeable in facial anatomy. He left me with an AMAZING nose. I liked the vision of my nose we agreed on at my consultation already, but after my splint was removed...WOW. I can’t even explain in words how amazing my nose turned out and how thankful i am. It looks EVEN BETTER than I expected it to turn out!! I’ll let the pictures speak for themselves but I just want to conclude by saying that I’m so happy I went through with this procedure and I’m so glad I chose Dr. Apostolakis to perform my nose job!
After seeing such amazing results on RS with tear trough implants I decided to look into it. I’m scheduled for surgery on June 19th and I can’t wait. I’ll try to post some pictures of before and after closer to my surgery date. I’m going to fly to Austin for the surgery and have already had my blood work, EKG and consultation. Updated on 24 May 2018: I won’t make decision on the lip implants until I have face to consult. Both the doctor and his staff have been very open and forthcoming so far. Very pleased with the doctors openess about discussing risks and possible complications that while rare do happen. I’m just so excited to be done with fillers every six months. I will post a before of my lips soon. No filler for a year so I’m really seeing how bad thy are! Lower lip implant is a liittle out of place so I have to try to keep it in place. Pain isn’t that bad, Tylenol is enough. It was nice to get the tubes out of my cheek. The doctor said my previous doctor had removed fat under eyes. I wish he hadn’t. Still up in the air about how well it’s going to look when swelling goes down. But it’s the first day and I’m not feeling well so I just need to be patient. Updated on 25 Jun 2018: Still eating Jamba Juice and you yogurt. My lower lip implant won’t stay where it should and is sagging on my right side. Not sure if it’s swelling or implant. Painful to own mouth because of tear trough implants. If I had just had lip implants this would be really quick recovery. Swelling is finally starting to go down. Bruising is a bit less and pain Hs been minimal. Tylenol is more than sufficient. I know it’s only day 4 but it feels like a month. I think when all is done I’ll be happy with the results. Updated on 28 Jun 2018: Still bruising and swelling but much less. The healing has actually been much faster than expected. My doctor and his staff were amazing and operate like. Well oiled machine. I thank them all for their expertise and emotional support! Updated on 13 Jul 2018: Still swollen but it’s slowly going down. I think I may need to go and get a 5mm put in my upper lip. Volume is still very slight. Bruising is slowly going away. Healing is going well. So far no complications and happy with the results. Updated on 24 Jul 2018: My lips are 4mm and I think I should have gone with 5mm on top. Little numb around my mouth still but no pain, bruisings gone and just a bit chapped. My tear trough implants are healing. Still swelling but it’s getting better. Bruising is almost gone. For some reason my right side I more tender and more swollen than my left side. My nose has been numb for weeks and I’ll starting to get feeling back now. Overall I’m happy with results. It will be interesting to see how I look with makeup as compared to fillers. Updated on 4 Aug 2018: My bottom lip is still bit asymmetrical but with makeup it’s not terribly noticeable. Thrilled with the results of my eyes! So glad everyone shared their experiences so I was able to learn about tear trough implants! I worth it. No more juvederm every 7-8 months. Still have. Bit of swelling on side of my nose but not bad. It’s goin to be another to months before it’s totally gone.
You have what I would call moderate tear-troughs. Injections of Restylane work very well for mild tear-troughs and in fact is generally all that I suggest. Moderate tear-troughs typically experience a nice improvement that is well worth the effort, but rarely is considered a complete correction. Severe tear-troughs typically just don't get a worth-while result from injections. Moderate and severe tear-troughs both do well with surgery. Between 1/2 and 1 cc is appropriate for each eye. Occasionally 1.5 if the treatment is extended all the way across the lower eyelid. If you need more than that, you need something else (surgery). Too much filler does not look better because the root problem is lack of bone structure and filler does not behave like bone. Hyaluronic acid fillers (Restylane) tend to last a very long time in the tear-trough. In fact, many years are possible in some people. I have done surgery on people who are more than 2 years out from an HA injection and during surgery, I find it in tissues.
Hello,You appear to have levator ptosis on the left upper eyelid. The root cause of this is a poor connection between the eyelid opening muscle and the eyelid structure. With this stretched connection, the muscle "tries harder" to open the eye pulling back deeper into your eye socket, drawing fat and skin deep into the eye socket along with it. That causes the long, hollow eyelid seen on that side. I can not see your eyebrow on the left, but commonly that eyebrow will try to "help out" by raising higher. As the connection between the muscle and eyelid structure worsens, the muscle will run out of room to pull back further and the eyelid will no longer open all the way. This condition can be congenital (born with it), trauma, or age related. People who wear contacts get it more commonly and the assumed mechanism is trauma related to placing and removing the contacts. Surgery can correct this condition and there are two main approaches. The one accesses the connection from the backside of the eyelid. This is a quicker easier approach with no visible scars, but has a lower success rate. The external approach is more powerful, but requires a scar in the skin and has more healing.
When it comes to facial volume injections, it is my opinion that the best option is a hyaluronic acid product. These include many brands and "thicknesses", I use Voluma and Lyft for cheeks. I am not in favor of using Sculptra. Serious complications are vastly more common and very difficult to treat. A well known complication is granuloma formation which may need to be surgically removed and can last decades. Scultpra relies on your body reacting to the product to produce any result. This reaction often simply doesn't happen, at least from an aesthetic perspective and if it does takes many months to appreciate. Sculptra itself is not a filler, only your body's reaction to it causes filling. Radiesse is a reasonable product. However, it can not be dissolved on demand like a hyaluronic acid. Radiesse produces an immediate filling affect and dissolves over time. Hyaluoronic acid products also produce an immediate result that slowly diminishes over 1 to 2 years. It is dissolvable on-demand and its chemistry (hyaluronic acid) is a material already in your body.
This all depends on what filler is used. The vast majority of fillers used are made of something called hyaluronic acid (HA). Juvederm and Restylane are the two most common brands of HA. These products will dissolve in time and will not lead to jowling or sagging. HA doesn't migrate, you can "push out" the edges with massage early on, but it won't altogether move to a different location. HA fillers can be dissolved on purpose when using a product to dissolved it (hyluronidase). The dissolving is very quick in this case taking only hours. Otherwise, HA fillers last from weeks to a few years depending on the location, the patient's genetics, and filler brand chosen. Other fillers are permanent and, in my opinion, should be avoided. Silicone is an example. It is legal in the US because of its usefulness in treating detached retinas. People (doctor and other) use silicone "off-label" for cosmetic injections despite a history of complications and FDA warnings against its use in that manner. My recommendation is to stick with using HA fillers and a doctor who is talented at injecting them. HA fillers produce tremendous results for quite a number of problems. A reputable injector will know what sort of results you can expect with your specific concerns. The key is to select your battles, some problems can be treated with great success, others not so much. An honest injector will steer you away from treatments that are not likely to succeed so you can focus your efforts on successful treatments.
Hello,An exam is needed to really define what needs to be done, but based on your photos and my experience, I am expecting that your nose is very soft and has thick skin. To provide a narrower, more refined nose, I'll need to strengthen your nose. Narrowing techniques by themselves won't work when applied to a soft framework. The thick skin can't really be changed, but a good underlying structure can largely compensate for the loss of definition that thick skin imbues upon your nose. To strengthen the nose, grafting will be needed. Presumably you have good septal cartilage and this will be useful in creating tip supporting grafts. Your columella is a bit retracted as well, and the same grafts for tip support can be used to push the columella down. While your nasal base is a bit wide, your nostrils are quite small and I don't think you should have base narrowing performed. Narrowing the upper two-thirds of your nose will require making osteotomies (bone cuts) on each side so that the side-walls can be moved inward. In summary, you need a great deal of tip support, narrowing of the tip structures on top of that support, lowering of the columella, and narrowing of the upper two thirds of your nose. Expect it to take many months, even a couple years, for 100% swelling to resolve. With a good strong underlying support, however, the result will then persist for a lifetime.Dr. A
