Breast Reconstruction by Dr. Leveque - This patient came to see Dr. Leveque after having a double mastectomy due to breast cancer. The patient also underwent radiation therapy for treatment of breast cancer; therefore, the latissimus dorsi flap procedure was appropriate for her breast reconstruction. Dr. Leveque placed 339cc silicone gel implants at the same time as the lattisimus flap. She later underwent fat transfer from her abdomen to her breasts to help create fulness in both breasts. She also chose to have nipple tatooing done for a more natural breast look.

18-24 year old man with Poland Syndrome treated with Breast Reconstruction with muscle flap - Poland Syndrome

A 21-year-old plastic surgery patient presented to our office for consultation regarding correction of a very rare breast/chest deformity called Poland Syndrome. He was otherwise very healthy, athlete. After through clinical evaluation we found that his deformity is mild to moderate and that he is only missing a portion of the chest muscle (pectoral muscle, peck muscle). More severe forms of this deformity include loss of the ribs with chest deformity, as well as deformity of the upper extremity. We discussed complex reconstructive breast procedure which will include transfer of a live muscle from his back to replace missing pectoral muscle.

His surgery was done in hospital in general anesthesia. It involved preparation and transfer of a powerful latissimus dorsi muscle from his back to the front; muscle was fixed to the ribs in the same position as a missing pectoral muscle to create symmetry with opposite site.

Patient is shown before surgery on the left; notice severe asymmetry between left and right breast/chest. On the right patient is shown 3 years after surgery; he was able with exercise to build transferred muscle to match the size of a contralateral muscle. He has a small scar in the right armpit which is the only reminder of previous condition.

55-64 year old woman treated with Breast Reconstruction - Patient is a 57-year old Caucasian female, initially referred to me by her medical oncologist.  I initially saw patient in consultation regarding possible breast reconstruction options.  She had been recently-diagnosed with diffuse, high-grade DCIS of her left breast.  Of note, she had a history of intraductal carcinoma (ER (+)/ PR (+)) of the right breast for which she underwent a mastectomy with axillary lymph node dissection.  She had a lengthy breast reconstruction at that time using a right latissimus dorsi myocutaneous flap and small implant.  She had some type of operative revision of her breast reconstruction.  She denied receiving any radiation therapy. She then underwent left total mastectomy with axillary lymph node dissection, and I then performed immediate reconstruction with a submuscular Tissue-expander that was filled with 100 cc sterile saline.  She complained that her old right, reconstructed breast was too small and didn't look right.  At the same setting, I performed a revision of her right breast reconstruction that had developed severe capsular contracture and deformity, and was also much too small in size in comparison to patients left breast.  I approached the breast via an inframammary incision, performed Capsulectomy and Capsulotomies, and removed a small, smooth, round 90 cc silicone implant that was intact.  I then placed Alloderm and an Allergan Tissue Expander with a total fill-volume of 500 cc into the pocket and filled the expander with 70 cc of sterile saline.  She went on to heal well and had tissue expansion performed on an outpatient basis.  She then underwent Removal of her Tissue Expanders and Replacements with Allergan (Natrelle) silicone gel, Style 20 (high-profile) implants- 500 cc on the left side and a 550 cc on the right side.  Finally, she had Bilateral Nipple-Areolar Reconstructions with local flaps and did not desire tatooing.
