Options for Improving Fat Above Nasolabial Folds?

I'm 25, I've lost over 25 kg in the last year. My cheeks have sunken but the fat above the nasolabial fold has remained giving me an older, tired, droopy look, especially in the morning. I especially don't like the fat above my nasolabial fold. What are my best and safest options to correct this one way or another? Should I do something about the cheeks or fat? For long-term results, should I opt for surgical and/or non surgical procedures? I'd prefer something thats more permanent instead of temporary fillers. Thank you.

Nasolabial folds after large weight loss

The nasolabial folds after large weight loss result from a deflation effect. They are best treated by augmentation of the cheeks and nasolabial areas themselves. In our practice we prefer untraumatized autologous fat-fascial grafts for this purpose. In some patients, direct excision of the nasolabial folds can be performed. However this is seldom performed due to visible scarring.

How to address fat above nasolabial folds

The best option for excess fat in the flat area of the cheek and nasolabial area is to put cheek implants in. These are wafers of silicone and come in four different sizes and can be placed in through the intraorbital route and from the bottom side lifted outward to volumetrically improve the cheeks. Cheek implants can be removed if the patient does not like them or later prefers a different size depending upon the patient’s needs, wishes, and concerns.

Treatment of excess cheek skin

It is likely that the significant weight loss you describe has contributed to the problem. The photo clearly shows what is bothering you. When you were heavier the cheek skin envelope expanded to contain the larger amount of underlying fat. After the weight loss there is less fat present but the skin has not shrunk down to adjust for this. The answer then is to remove some skin and/or add some bulk under it. Facial deflation

I assume the weight loss was a healthy one for you. It is nit uncommon to note deflation of the facial soft tissues as you lose weight. This causes the hanging of the skin and the contour irregularities your see in the photos. One test I have patients do is to do a half-smile and see if this doesn't round the cheek area out sufficiently. If it does, I might recommend a particular midface lift from inside the mouth.

It is hard to tell from your photo, but it appears that you have some facial soft tissue atrophy. Sometimes facial fillers can help soften this appearance.

Deflated cheeks following weight loss

I would address this with a combination approach. Fillers or Autologous fat to the cheek and Nasolabial folc could help as the least invasive procedure. Alternatively a cheek lift with implants would likely give you the longest lasting result but may carry the highest risk

The treatment for facial depression of the face after weight loss.

It appears from the photo which is sub-optimal that a combination of cheek lift with cheek implants would be best. See an experienced facial surgeon and he can examine the face and tell you what is best.
