You ave very reasonable support of the jawline region and minimal cheek "heaviness". The only really effective option is a true neck lift with platysmaplasty. It is essential that the entirety of the muscular plane under the skin be re-supported booth laterally and medially in order to achieve long lasting smoothness, support, and a natural result. Other treatment options would likely be disappointing including RF or Laser options.

You have a relatively common area of concern with both a pocket of fatty tissue as well as a slow loss of overall support in the under chin neck area.
These are often genetically predisposed areas often "running in families". There are several options for treatment. The most definitive is a true neck lift /platysmaplasty combination. This fundamentally restores the support of the entire region and also eliminates the pocket of fatty tissue. We are also able to achieve smoothness and a very natural outcome. Beyond this there is the option of liposuction which is easily accomplished and offers a likely significant improvement but cannot address problems of fundamental support. Other options such as cool sculpting are very limited and only offer a limited reduction. Kybella is probably the least attractive option. If you are seeking a true neck lift be certain that the technique includes restoring muscular support both medially and laterally in order to achieve a lasting and excellent result.

ThermiTight is a very reasonable option in selected areas. Our experience has been very positive in treating otherwise difficult areas not easily treated by other modalities such as liposuction. Examples would be the inner thigh region, upper arms, breasts, abdomen, and facial areas. We do find that the best results are those who have a small amount of fat as there is in fact a degree of "melting" that takes place. Most importantly is the skin tightening that is associated. We have had surprisingly good results in essentially "non-surgical" breast lift. The inner thighs respond well with mild fat reduction but also skin tightening. Fat that is reduced would be essentially permanent as in liposuction.

Changes in the brow position have a profound impact on overall appearance and our perception of an individuals age. There are often strong genetic factors in the brow position and the degree of "droopiness" seen. We have seen a number of individuals in there 20's who have significant droopiness that results in a "tired", "sad", or fatigued appearance. The upper lids often will appear heavy or redundant as the brow descends. I believe the endoscopic technique give us excellent and essentially permanent results that are totally natural.
With some modifications of technique we see patients having very presentable appearances in as little as 3-4 days.
Ultimately, this is one procedure that may be entirely appropriate even at a young age.

Our preferred technique is the endoscopic approach which when performed properly achieves consistent, reliable, and very long lasting results. It also essentially eliminates any visible signs or widened scars that are common with other approaches.
This technique also essentially eliminates the "deer in the headlight" appearance that seems to be common with some other techniques.
There is very little trauma and in most cases very little bruising or swelling.-The key is to find a surgeon with a large experience base in the endoscopic technique and you should achieve great results.

The face lift procedure can actually do little to eliminate marionette lines as these are more related to the inherent loss of collagen and elastic fibers. In fact when surgeons try to correct this area aggressively generally by pulling skin this can lead to the "joker smile" type of appearance that is then very difficult to address.
Ultimately the options include judicious use of fillers, RF procedures such as Venus Freeze or Thermi smooth,and more recently the use of "Neo-Threads" which are absorbable threads providing support and smoothness.

The most important thing to look for is that the images are as close to identical as possible in terms of positioning, lighting, color balance, and overall brightness and contrast.
It is important that facial expression is neutral and similar in both. Far too often surgeons may have a before image that shows a non-smiling expression often with the chin a bit lowered and then the after image is smiling with an extended chin which dramatically changes the parameters of the comparison.
Look for sharply focused comparisons not an after picture in which the contrast has been reduced and the brightness increased to reduce the viability of details.

It is in fact difficult to correct as the initial cause is related to an over-pulling of skin in an unnatural vector instead of focusing on the underlying tissue support without "pulling" the skin. It is difficult to recommend other options without some images-non smiling.
It may be possible to reduce some of the appearance with filler or possibly "neo threads".

At 4 weeks I would expect an overall better appearance. It appears that the incision is quite red and reactive. There may also be some (probably temporary) hair loss. You should consult with your surgeon regarding this. There are a number of options to promote better healing such as addressing any potential inflamatory reaction as well as treatment with "V-beam" therapy.

Ultimately, the timing is an individual choice. Your images show some degree of platysma banding and laxity which is in fact very correctable with potentially even better results than your prior lift. If it is reasonable for you I would encourage revision which should then result in significant longevity.

A couple years ago I realized I did not smell food properly. A friend recommended I see Dr. Ludwig Allegra for sinus problems. He did a CT Scan and found I had a lot of blocked sinus passages. While under sedation for sinus surgery I added a face-lift. Both went well. Recovery was easy – one week, no pain. I look younger and can smell again. He fixed my silicone breasts that were placed to far apart. The first Dr in Kirkland that did the implants would not fix them. I had cross eyed nipples and implant that slipped into my armpits. And she said this was normal and would not correct the problem. Dr Allegra repositioned my implants and corrected the outside edge of my pocket so the stay out of my armpits. They are perfect now. The implant is centered under my nipple and they are beautiful. He is a fabulous surgeon.
My experiences with Dr. Allegra were very productive. Dr. Allegra has performed 3 procedures on my me. The first in my late 20's, I had monthly sinus infections and had to pay out of pocket for the prescriptions due to some complications in insurance. My primary doc, Dr. Mary McGregor, said no more prescriptions until I saw Dr. Allegra, she called my hand. He took a look and scheduled surgery without hesitation. Within a couple of weeks I felt like a new person, I had energy, I didn't take a 4 hour nap after work, I could exercise, life was great. In my early 30s husband and I moved to Michigan for job advancement. While there I was exposed to black mold, it was in our home. I kept getting sicker and sicker, no medical person could figure out why I was sick. I will say the symptoms were odd and complicated.
