Biggest determinant wis amount fo fat/volume relative to the amount of skin excess. If the main problem is fat and it is lovalized to either inner thigh or outer thigh or both and the skin relatively tight, then liposuction alone may help.
If there is significant bulk.volume, then better to stage this- first stage is to do liposuction (ultrasonic or laser liposuction) to deflate or debulk the thighs; then 2nd stage, a thigh lift where the excess skin is excised.
Thigh lifts can be done in different ways. Medial thigh lift, wherein scar is horizontal vs an extended one wherein scar may be horizontal + vertical, will give a more extensive scart, but more skin/fat can be removed.
As always, it is best to consult with a board-certified plastic surgeon and it may even help to visit a few so you can benefit from, possibly, differing opinions.
