Thank you for your question. It is very possible that one of your medications is causing the uncontrollable diarrhea. See if the stopping of your medications helps to resolve it. You should call your Board Certified Plastic Surgeons office and keep them updated on your status and if it does not stop soon I would ask them about recommending an over the counter anti-diarrhea medication and voice your concern that it can lead to dehydration.

Congratulations on having undergone the breast augmentation procedure. I ask my patients to wait 3-6 months before evaluating the end results of the changes in their breast size and shape as well as their overall new body image. There's a significant percentage of patients at your stage of recovery that feel that they are too big or (more commonly) too small overall or on one side. And those emotional feelings can change daily turning this transitional time. Remember also that each side of the body heals at a different rate and that you weren't perfectly asymmetric to begin with, there will always be differences. You need to let all healing resolve completely before you can make that determination. Once the swelling around the implants has also resolved you can see the final result. Patients often report that they are happier with the implants' shape, their cleavage and projection once these changes occur.

I would suggest continued patience and continued close follow-up with your plastic surgeon. for an outcome that you will be pleased with long-term.

The surgeon would meet with you and examine you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of the operative procedure in person and what to realistically expect. Then you can make an informed decision on size and shape. Your surgeon should be able to explain to you why they are recommending the size they are according to your current breast measurements including your chest width and your desired outcome.  

We have all our Breast Augmentation patients not only utilize our 3D imaging so they can get a real visual on what the implants will look like on them but also to actually try on the different size implants with a tight bra and t-shirt to see what they might expect. That way they have a good visual of what a 450cc implant will do as opposed to a 350cc implant for example. You want to make sure that you see a surgeon that will listen to you and your goals for the procedure and discuss in detail their recommendation for size and shape not only taking into consideration your desired outcome but also your breast measurements before the procedure including your chest dimensions. And then let you try the implants that they feel will give you the desired result so you are in agreement with the size.

Then you can make an informed decision on which procedure(s) to start with according to your desires, recovery time necessary, your history and current health. Surgery after significant weight loss is usually very rewarding for our patients that have already worked so hard to loss the weight and a great journey to be on with our patients. 

One of the most important factors is not only your health going into the procedure(s) but the support system you have available for your recovery time and the amount of time you can seriously devote to recovery. If you have three active children and only a few days where you can rely on assistance driving, taking care of housework and children and daily activities then staging the procedures may be your best option. If you have help for a good two weeks where you can really focus on you and your recovery and someone else is going to take care of the daily routine and responsibilities than it is not uncommon to do combined procedures for one surgery, one recovery. Patients that have the needed recovery time scheduled in advance usually recover faster as they don't have the added stress of knowing they are needed 4 days after a surgery to be back to their normal routine. Take all of this into consideration with your surgeon when making this decision and get realistic answers to your concerns in advance will make the whole process easier. Before you make your decision on a procedure(s) I would suggest that you meet with a Board Certified Plastic Surgeon again and go over your concerns and what you hope to accomplish.  Your surgeon should be able to explain to you why they are recommending the size and procedure they are according to your current breast tissue, measurements including your chest width and your desired outcome. Photos would help with answering your questions.  A breast lift will give you a more youthful shape and a fat transfer to the breasts can be done giving you more fullness but it does have its limitations. Your Surgeon should be able to advise you thoroughly on your options.

It can be hard and confusing for consumers when a physician may tell you they are Board Certified. Make sure it is Board Certified in Plastic Surgery as many Physicians are certified but it is in another area such as Dermatology, Gynecology, ENT, or some other specialty that does not have the years of training and Certification in Plastic Surgery required to be a Board Certified Plastic Surgeon.

And those emotional feelings can change daily turning this transitional time. Remember also that each side of the body heals at a different rate and that you weren't perfectly asymmetric to begin with, there will always be differences. and other factors must resolve completely before you can make that determination. They may seem completely different at week 6 and by week 8 much more in symmetry. Once the swelling around the implants has also resolved you can see the final result. Patients often report that they are happier with the implants' shape their cleavage and projection once these changes occur.

I would suggest continued patience and continued close follow-up with your plastic surgeon. for an outcome that you will be pleased with long-term.

Your desire for a pointed chin is achievable, and you could start with an injectable filler that isn't permanent to fill in and define the areas you wish to change and see if you like the look before you decide to have a surgical procedure with a more permanent result. The pointed chin implant is possible but would require a customized chin implant. Most implants are designed to achieve overall fullness and projection to recessed chins.

It would help to see full facial photos (front and sides), but yes, refinement of the hanging or low tip is possible even if you're happy with your dorsum (the slope part). Assessment of the nose includes examination of the cartilage and bones, frontal/base/side views, intranasal exam (inside the nostrils to look at the septum), and evaluation for symmetry. Sometimes releasing the depressor nasi septi muscle helps rotate the tip upward slightly so that it does not plunge with smiling. Care must be paid to avoiding overtreatment since you like your nose in repose. We use Crisalix simulation software to help guide the discussion, as it lets you see a 3D model of your face with potential surgical changes applied. Go on some consults with board-certified plastic surgeons and/or ENT surgeons with facial cosmetic expertise.

I had an explant surgery with Dr. Sayed in August 2024, and I cannot say enough good things about the experience and the results. I was dealing with a grade 4 capsular contracture, which had caused me significant discomfort and visible distortion. Changed my Life for the Better!

Dr. Sayed changed my life for the better. During our first meeting he address all my concerns. He took his time to answer all of my questions for my breast implant removal surgery. I also decided to do a breast lift and liposuction. Scheduled surgery and everything went smoothly. I felt very confident in his team during the process. My healing and current health has been such a huge positive in my life

From the time I met Dr. Tim Sayed in our face-to-face consultation, I felt instantly relieved and assured that my concerns about my breast implants and my health were taken seriously and acted with great consideration for what I was going through. I knew from that first meeting that he was the right surgeon to perform my explant surgery, especially when he asked if I was mentally prepared for the change along with the physical changes that would come with explanting. Anyone who has gone through this particular journey knows that it’s not just a physical game, it’s a mental commitment as well. My first augmentation surgery was done in 1994, I had a single implant rupture and full replacement of both implants done in 1998. Those implants were saline, and I really never connected the dots of some of my health issues at that time, because life was happening at full speed with starting a family. After all those years of having them in, I decided to do the responsible thing and have them switched out with my original cosmetic surgeon for a new set of implants in February of 2023. My saline implants were replaced with silicone, they never settled properly (I had bottoming out and double bubble effects), and from that point, my health issues started to spiral. It may have been because the capsule was left inside from doing the implant replacement, it could have possibly stirred up some issues & also potentially with the new implants being silicone. My original surgeon didn’t necessarily feel that I was having breast implant illness issues, and said he would fix the complications of the implants not settling properly. Many other health issues happened in between and gratefully that revision surgery never happened, and instead, I went on a quest to find a surgeon who would be in the space of understanding my health complications, my uncomfortableness physically with the new implants and work with me on my explant journey. My explant surgery with Dr. Sayed took place on September 30 of 2024, and my healing and current health has been such a huge positive in my life, not saying that surgery is easy to come back from, especially in your later 50’s, but so worth it and I have zero regrets. I feel better than ever. Dr. Sayed and his team are stellar. His attention to detail, embracing holistic healing methods (I do currently work with a naturopathic functional nutrition practitioner), the time he takes to explain and have actual conversation with me as his patient means everything. I believe this is the new face of cosmetic surgery where doctors are actually fully committed to not just the aesthetics but to the whole health of their patients. Discussing nutrition, supplementation, exercise, mental health and more are such important elements. Those elements, along with skill & experience, in concert, are such an important part of making a decision as big as explant surgery. I’m very pleased with my results at the almost eight week mark & I know it’s just going to get better. I can finally breathe! Finding the right doctor for you can be extremely stressful and nerve wracking, not at all with Dr.Sayed. The moment you schedule a consultation with him your sold. His bedside manner as a doctor is top notch. Unfortunately we don’t usually find this with many doctors. I’m 7 weeks post op and have not had one problem or complications . Excellent Doctor and staff…I had an implant reduction and a lift with Dr Syed a few months ago and am very happy with my results! Dr Syed is very thorough and informative…his staff was great! I highly recommend him to anyone looking for excellent care from start to finish!
