There have been a significant number of complications due to injection of Botox and fillers in a non medical setting, many of them never reported.

Botox is a medical procedure that needs to be performed with sterility, using proper technique by a trained physician.  It may also be injected by a qualified licensed practitioner under the supervision of a physician depending on the state laws. 

The medication used must be approved by FDA and the physician needs to be very familiar with the muscular anatomy of the area treated.
