48-year-old with implant reduction and breast lift

I've never liked the shape or appearance of my breasts. Then after nursing two children, it seemed like the more fit I became, the worse my breasts looked. I couldn't afford a breast lift at the time, so in 2004 I got sub-muscular saline implants. I had always been athletic and never wanted big boobs but the surgeon went with larger implants than I wanted (probably due to the amount of sagging). And the left one has always been a little "wonky".lol
My implants are too big and they've don't sit quite right. My nipples point downward now (even more so on the left). I especially hate the "animation deformity" I get when my pectoral muscle flexes. It's surprising how often that happens!
Now, nearly 18 years later, I'm ready for a breast lift. I contemplated having the implants removed completely but have decided to replace the 350cc saline implants with smaller (160 or 190cc), silicon gel ones.
Dr. Vath at The Center For Cosmetic Surgery in Golden, CO will be performing the surgery. The consultation and pre-op appointments were great. The doctor, nurse and patient counselor have all been wonderful so far. They have all been super helpful and very knowledgeable!
17 days till surgery. The only decision I have left to make is whether to have the implants placed under the muscle again, or to have them placed over the muscle. I assume that with a lift and smaller implants (under the muscle), the animation would be "better", and I do worry that going over the muscle will look more unnatural... but I really hate the animation! I'm still on the fence...
Surgery date is Feb 23, 2022.

First of all, you look great! I am 47 and I had a lift (no implants, large breasts already) May 2020 and it was the best thing I ever did! The immediate relief was amazing and now I have the breasts I always wanted. Wish I would have done it 10 years ago! Good luck, you're gonna love it!

I can’t say enough good things about the Center for Cosmetic Surgery in Golden, CO. From the intake nurse to the anesthesiologist to the surgeon… this place is top-notch and they run it like a well-oiled machine.
Dr Vath has lovely bedside manner and from what I can tell so far, he did a great job and everything went as planned. I also had less pain than I expected and was able to work from home the day after surgery. At the time of these photos, the swelling should be at its highest (according to the cool chart I got from the surgery center).

Forgot to mention that I decided to have the new, smaller (160 cc) implants placed under the muscle again.

Thank you so much for posting! I’m in a similar situation - 350 cc implants currently and looking to downsize with a lift. My surgery’s in 2 weeks and for some reason I’m really nervous. Your post op pics look great - I can’t believe they were at 3.5 days post! Now that it’s been a few months is there any advice you have? Anything you would do differently?
