Answer: Restoring the midface Hello, Thank you so much for your question! Without pictures or an in person consultation it’s hard to say but from what you seem to be indicating filler,fat transfer, or cheek implants may accomplish your treatment goals. In that order from least to most invasive these are all great ways to add volume to your midface! If you previously had your zygomatic reduced then I would treat you like someone that didn’t have strong zygomatic to begin with a great plan for augmentation! When it comes to deciding on treatment my best recommendations are to meet your prospective surgeon, check out their facility, review their photo galleries either in the office or online and lastly if you feel the need speak to their prior patients. An XRAY (standard facial series) should determine the extent and position of the fractured segments. These segments will need to be secured and fixed, to one another as well as the temporal and malar bony portions of the face, using metal plates and screws through incisions in the temple and inside the mouth. You should seek an experienced ENT surgeon in your area for the evaluation and repair. Dermal fillers or silastic cheek implants combined with liposuction when excess fat exists is the appropriate method of shaping the cheeks. It's best to have the zygoma repaired first (if a repair is possible), allow the tissues to heal for 6 - 12 months and then consider silastic cheek implants if your cheeks are flat or concave in the front. The possibility of having silastic cheeks implants will depend upon where the metal plates and screws are located. Hope this helps.
