Chin dimpling and sagging after silicone implant removal. What can I do to improve this ugly conditions? (Photo)

The chin dimpling you're describing results from an overly active mentalis muscle in the chin.  I often see this mentalis hyperactivity in people who have a weak chin and need chin augmentation (in this situation, it's called mentalis strain and likely results from the mentalis muscle working extra hard to keep the lower lip up and the lips closed).  It's also not unusual to see this peau d'orange, or pebbly chin, appearance in people with normal chins.  Chin dimpling will often increase after removal of a chin implant, as the mentalis muscle has likely been disrupted with your several surgical interventions.  I find Botox to be very helpful in all the patient groups described above; it works very well for mentalis hyperactivity, and may provide you with a very satisfactory result for the dimpling.  Botox will not do much for a saggy chin (called a witch's chin deformity); however, your photo, while limited, doesn't suggest you have this.  Likewise, Botox won't do much to the scar you appear to have on your chin: if this scar is depressed, it can be improved with a surgical revision and/ or subcision with possibly a little plumping agent or filler underneath.If you have a remaining depression or two after trying Botox, these may certainly be addressed with filler--whether a hyaluronic acid like Restylane or fat grafting as mentioned previously.Finally, the patients who complain of a "too-round" face sometimes have masseter hypertrophy, which results in an overly angular or full jaw.  If this is your issue, Botox to the masseters may also work quite well.  Continue discussing your options with your surgeon, and good luck in your treatment!

Unfortunately this is a complex problem not just because of the information you gave but also because of the scarring that appears to be present. A more definitive answer can only be given after a thorough history and examination.  Your best option to restore volume is micro fat transfer also referred to as fat grafting or fat injection. Treating with Botox or a similar agent just before the procedure may help the fat heal in place better. It is important that you consult a surgeon very experienced with fat transfer. 

I recommend layered fat grafting to eliminate the ripple effect. Only few PSs offer this operation.....
