You have cutis pleonasmus (excess skin) and dermatochalasis (loose, redundant skin) on your medial (inner) thighs. The most effective treatment for excess skin in this area is a thigh lift (thighplasty). Based on your anatomy and degree of skin laxity, the procedure that is most appropriate for you is a crescent thigh lift. During a crescent thigh lift, an incision is made within the infragluteal fold, where the buttock meets the back of the thigh. The incision is then extended forward into the groin, following the natural crease between the pubic region and the labia, and continuing along the inner thigh crease. This incision placement allows excess skin and soft tissue to be removed while positioning the scar within natural skin folds whenever possible. An important part of the procedure is securing the lifted tissues by anchoring the incision to the pelvic periosteum, the strong connective tissue layer that covers the pubic bone, using permanent sutures. This fixation provides additional support to the repair and helps reduce tension on the skin closure. Anchoring the tissues to the pelvic periosteum also helps minimize several recognized risks associated with a crescent thigh lift. Specifically, it reduces the likelihood of downward migration of the scar onto the inner thigh, decreases the risk of widening or spreading of the labia (labial spreading), and helps minimize enlargement or distortion of the vaginal opening (introitus). Although these permanent suspension sutures significantly reduce these risks, they cannot eliminate them entirely. The crescent thigh lift is performed under general anesthesia and takes about 3 to 4 hours to complete. Allow yourself a recovery period of 3 to 4 weeks.
