Basically you have two options. Well three actually. When would be a medial epicanthoplasty which would require an incision and the scarring from which is almost always at least a little bit visible. The second would be incision take me with extension of your existing crease further medially to create an open or parallel crease. This could do a nice job of opening the inner corner and making a chopper.finally, there is suture technique to create the same parallel crease extending your existing crease towards the inner corner. You would benefit from an in person evaluation of your eyelids. It seems you may have some ptosis on one side.
