Would an under eye fat transfer without lower bleph be effective? (Photos)

Hello, I recently had my first consultation with a surgeon in Germany who specialises in fat transfer and eyelid surgery. I expected I might need a lower blepharoplasty, but he advised against it. He said the fat under my eyes is not prominent enough to justify removal/repositioning, and that this could risk creating more hollowing. Instead, he suggested subtly improving the lower lid-cheek transition with volume — around the tear trough/lid-cheek junction, outer hollow border, and upper medial cheek beside the nose — to help the area catch light more harmoniously rather than dramatically change my face. For context, I already have a small amount of cheek filler: approx. 0.5 ml per side. He did not recommend dissolving it and suggested working around it. His initial recommendation was an off-label/proprietary combination of nanofat with a small amount of Sculptra. After I voiced concern about Sculptra in the under-eye area, he said fat/nanofat without Sculptra would also be possible. Based on my photos, does this look like a case where subtle fat transfer/nanofat to the tear trough and medial cheek could work without lower blepharoplasty? Given the existing cheek filler, would you dissolve first, use structural fat/microfat, nanofat only, or a different approach? Thank you.
