Do I need genioplasty? (Photos)

Question

Hi, I'm planning on getting rhinoplasty to even out my profile and reduce the size of my nose. I think my jaw is fine but I am unsure if genioplasty will make a difference.

Without an exam your question cannot be answered.  You will need to see an Oral/Maxillofacial surgeon to see if othognathic surgery, genioplasty or using an implant is best for your specific case. It should be noted that patients with severely weak jaws or abnormally functioning jaws may be better treated with other facial surgery (orthognathic surgery), although a chin augmentation may be an option at a later time. These patients should seek the advice of a plastic surgeon to determine which course is right for them. Here is some information to help you make this decision: Candidates for Chin Augmentation - Patients whose chin profiles are weak but have normally functioning jaws are candidates for Chin Augmentation. Many of these candidates have used injectable fillers in the past to strengthen the protrusion of their chins, but are looking for a permanent solution. If you are one of these patients, then you are a likely candidate for chin augmentation

A sliding genioplasty is a complex operation that isn't justified by the degree of chin recession you have. A small chin implant could bring your lips and chin into alignment instead. That, in combination with your rhinoplasty, will give you a stronger and more flattering profile. I hope this helps.

A formal genioplasty where your chin bone is cut and advanced forward and possibly inferioly can be performed at the time of Rhinoplasty. Your chin however is not particularly small and you could bring it forward slightly with a small chin implant or even an injected filler. When you have computer imaging during your Rhinoplasty consult ask the surgeon to also image your chin so that you can see the result with our without the change.Good luck

Many surgeons routinely recommend chin augmentation when having a rhinoplasty. It tends to create a better balance to the face. In most cases a genioplasty is not necessary and a simple silicone chin implant will provide the same result. Chin augmentation in your case will make a significant difference in the final results. Discuss these options with your surgeon.

Based on very limited information your photos provide and short of in-person evaluation, I think that you do not need chin augmentation  If needed you can do it in a later stage as a separate surgery. Discuss this option with your surgeon again and/or make another, second opinion consultation before your surgery. Good luck.

The limited side photographs demonstrate an overly projected nose, and an under projecting chin. As long as the teeth fit together properly, a genioplasty is not required. Consider a chin implant to augment the chin follower to help improve the projection of the chin to help with the side profile, especially with respect to the overly projecting nose. A rhinoplasty and a chin implant procedures can be performed together under one anesthesia with one recovery. A genioplasty is performed by an oral surgeon at a hospital setting with an overnight stay.  A chin implant with a rhinoplasty performed as an outpatient procedure. Your concept is valid and since the images are in differing head positions it is difficult to confirm. One needs to consider the height of the chin as well as the projection along with the inclination of your lower incisors. Balance is the key and even the cheekbone projection has to be considered. Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  From the photos, your chin is weak.  When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut bone segments).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.  I have frequently performed Rhinoplasty and chin implant surgery together.Hope this helps.
