Hello. What did this doctor use to change the nasolabial angle from acute to obtuse? Besides rotating the tip. The upper lip went toward the face before the nose and now it goes away from the face and into the nose. I like this effect. What graft can be used for this? I don't want fillers. Thank you

It appears that the patient has had a reduction in the poly-beak deformity along the bridge line,  A columellar-plasty and rotation of the tip upwards. Without looking at the operative report, it's difficult to say what exactly was accomplished during the procedure. For many similar examples, please see the link below

The after surgery has had cephalic trims with dome binding that rotate the tip upward and make the tip smaller. The cartilage of the rounded part of the septum on side view has been reduced. The medial crura look like they have been advanced on the septum and the lower edge of the septum may also have been trimmed slightly. The upper lip skin may be more tense because of being pulled or the surgeon may have used a cartilage plumping graft in this area.

Hi Rozeroze. In the preoperative picture (to the left), the patient has a pollybeak deformity, hanging columella and possibly mild alar retraction, and bulky infratip lobule. The postoperative photo (on the right) shows a nice result. There are several methods to address this columellar-labial angle, which often involves repositioning the cartilage of the tip of the nose. One method is a tongue-in-groove, which pushes the cartilage back onto the septum. The exact methods vary by surgeon, and in rhinoplasty there are often a few ways to achieve an aesthetic goal.  Fillers will not achieve this result. Finding a reputable surgeon who is an expert in rhinoplasty is a good start. Good luck. Best, Dr H.
