I have developed GERD after getting the gastric sleeve. Should I get a bypass to resolve the GERD? (Photo)

Question

I had a sleeve 12 months ago and have had serious GERD ever since. I take PPI once/twice a day to keep it under control. I have developed side effects from the PPI including anemia, dizziness, muscle cramps, stomach pain and headaches. I have had tests for colon cancer, gallstones/kidney stones, stomach/throat problems and they have found nothing. Want to stop taking the PPI but find when I stop the GERD is intolerable. Thank you. Conversion to a gastric bypass is a secondary option for treating acid reflux post sleeve. I would also rule out a sliding hiatal hernia a cause of your symtoms. Even a small hiatal hernia in the presence of a sleeve can cause severe reflux and a corrective operation can alleviate your symptoms.

The gastric bypass is the surgery we recommend for  sleeve patients who do not have hernia and have not been able to control GERD with medication.  
Considering another procedure may not appear to be  the optimal situation but GERD can cause serious problems long term if not treated. 
I would recommend consulting with a bariatric surgeon to explore your options.

Severe reflux after sleeve is traditionally treated surgically with conversion to a gastric bypass. Another alternative is the LINX procedure if it is available in your area.

Conversion is not always an answer to GERD, but more of a work up is needed.  Why you are having GERD may be because of the opening of the hiatus (where the esophagus goes into the stomach from the chest) - it may be an angle of the sleeve portion of the stomach to the opening. There are a number of factors.  If you just convert, you will have less of a high pressure system and the material will drain easier, but that does not necessarily remove GERD. So- see a surgeon who does these and go from there.
