breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.
Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.
While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.
If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Yes, the nipples can be more centered. This could be accomplished by either repositioning the implants, revising the lift, or both. To provide the best answer would require a physical examination. Ideally, you would discuss your options with your surgeon. Otherwise, seek a consultation with another board-certified plastic surgeon.

The optimum position for a nipple aesthetically is on the meridian of the breast (mid way between where the breast meets the chest wall in the midline, and the outer border of the breast) on the most projected part of the gland. Based on your photo, you have a long nipple to inframammary fold distance and there is a relative lack of volume above the nipple in comparison with below the nipple. This could be adjusted with a revision mastopexy, and the nipple position could be adjusted at the same time if required.
