Abdominoplasty scars can be quite complex, ranging from the simple hypertrophic scar to a multiplane inflamed keloid. Pigmentation, discoloration, chronic inflammation, scar visibility can all occur in the short term. You have some stretch marks visible and a small amount of skin excess when you sit. It isn't clear a full revision tummy tuck is needed, as you probably don't have much excess skin when standing. But if it is still lax, the result can be improved using the existing tummy tuck scar.
It does appear that you still have a lot of skin. You would benefit from a revision, probably entailing a complete tummy tuck again. You simple have too much skin left.
It appears that you are a good candidate for revision abdominoplasty. There is some excess of skin that may be removed; however, I am afraid that you do not have so much that one can remove all your striae covered skin. Would this revision need a transposition of belly button as well can be decided only on the basis of in-person clinical exam. Good luck.
Based on the photographs you provided, you may be a candidate for a revision tummy tuck to get rid of more skin. It is a little difficult to tell, particularly without seeing a picture of you standing upright. The issue may be that the scar is tethered down to the muscles, which is a different problem but requires a similar approach. Seek out a board eligible or board certified plastic surgeon to address your concerns - best of luck!
Thank you for your photos. It is best to have an in-office examination to determine if a revision will help get you the results you are looking for. From the photos alone, it appears that you may have some loose skin around and below your belly button as well as stretch marks. It is possible that your skin slowly stretched out since your surgery and it may be time for a touch up. You appear to have enough extra skin to remove much, but maybe not all, of your stretch-marked skin. Part of the reason I say not all is that you have stretch marks extending above your belly button even AFTER you had a prior tummy tuck that presumably removed a good amount of stretch marked skin. Sometimes there is not enough lower abdominal skin to remove all the tissue between the belly button and the scar position, which means you may need a small vertical slit incision scar in the midline between your belly button and scar when all is said and done (that's the hole that the belly button would be cut from but closed down lower after pulling the skin down). In-person exam is required to properly assess you. Liposuction as part of your procedure would likely be helpful in order to smooth contours and transitions from the abdomen to the pubic region and thighs.
In most cases, the inventive spirometer can be stopped after a few days but best to ask your plastic surgeon.
