Dear doctor, i ve a very UGLy NOTICABLE scar n ma nose wich is around 15 years old due to injury from a bamboo- it is very noticable n extremely decreses my confidence :'( i cant face people at times- can u please suggest techniques or procedures tu get it treated? i am a asian a female it makes me very self conscious- the scar is deep brownish in color- i just want it removed- please suggest me- thank u

Our treatments use combination therapy to improve the epidermis and dermis for most deep scars. The options available for acne scars depend on the character of your scars, in terms of topography and contour. We see atrophic and hypertrophic acne scars, as well as scars that have hypo and hyperpigmentation. We begin each scar evaluation with an assessment of scar quality and subtype. Our main concern is rebuilding collagen and that is why we use a HIDEF protocol that includes combination therapy. Morpheus 8, scar subcision, fibrous release, fractional laser, CO2 laser, Thulium laser, PDL Vbeam laser, and fractional resurfacing can be used in addition to TCA Cross and chemical peels to further improve skin. Patients should start Melarase AM and Melarase PM for active hyperpigmentation. Always begin with a consultation to decide whether early subcision should be started.

I would consider fractional nonablative resurfacing with Fraxel 1550nm which has been shown to be safe in dark skin and can help with the remodeling of the scar and improvement in the skin color and texture.  You could try laser genesis or fractional erbium resurfacing as well.  Seek a board certified dermatologist with expert training in lasers and cosmetic surgery. Since you are pigmented and the scar is pigmented the best treatment will be Laser Genesis, non-ablative 1064 nm. Look this up in Derm Surgery August 2006.
