Septoplasty failed? My surgeon says the swelling is causing my congestion. Is the swelling permanent or will it dissipate?

It can take several weeks to months to fully heal after a septoplasty and turbinate reduction. Sometimes the nasal lining (mucosa) remains inflamed from the surgery, and the healing time can vary between individuals, particularly if one has a history of allergies. If an oral decongestant helps you significantly, the obstruction is more likely related to mucosal inflammation/swelling rather than a structural blockage from bone or cartilage. The swelling should hopefully improve with time. In the meantime, a nasal spray may improve your symptoms.

It is still early in the recovery process, and I would recommend continuing to follow-up with your surgeon to assess your breathing.

Since you have had some improvement in your breathing, you will need to give the nose more time to heal. Some patients will describe the sensation as "opening up" well after 6 months.  Your surgeon may prescribe nasal steroids or inject the turbinates with kenalog to encourage the nose to open more quickly. If you are not pleased at one year then you could consider additional management but time may be all it takes to breathe better.
