Hello doctors, immediately after my transconjunctival fat repositioning & fat graft under my eyes, I could feel a very large hard lump under my right eye, and a smaller one under my left. My doctor said it was scar tissue and would soften over time. A year later, the lumps are still there and have really not decreased in size at all. Does it sound like scar tissue? What can I do to improve it?
Answer: Likely still fat Hello, if you had fat placed in this area and it remains the same, it is probably a combination of scar and fat. I would still recommend a little more time (6months), with massage and you may consider a small injection of steroid into the area; the other option is surgically removing part of it. Good luck!

Answer: Fat transfer to the eyes and lumps afterwards Dear edealee,The nodules could come from the fat transfer (most likely)It is a relatively easy fix with injections using kenalog and a medicine called 5 FUIt may take more than one round of injection, and you should wait 6 weeks between injectionsIf the nodules do not resolve, they can be surgically removed under local anesthetic (thankfully, this is rare)Best,Dr. Nima

During a transconjunctival blepharoplasty with fat repositioning, fat is expressed from the eyelid area and secured into the tear trough area. If you still have lumps in that area, it may be the fat. Answer: Lumps hi I am sorry for your issues.. you have been patient and waited a full year... if you still see lumps and bumps, chances are after one full year, they will remain as such.. I recommend going back to your Surgeon and getting update evaluation and decide what can be done to improve upon the present situation.... if you trust him but he feels uncomfortable as to the next step, perhaps he can recommend a colleague who is an “expert’s” expert who specializes in revision surgery to the lower eyelid region. Just few words of advice, there is no one best surgical technique out there no matter how good it sounds... the best technique that will yield a successful outcome to your eye is based on your anatomy.. meaning based on your Surgeon’s correct analysis of the quality of your skin, thickness and strength of your muscle, and the amount and severity of your fatbag.. very often transconjunctival fat repositioning is not feasible if the fatbag can’t be adequately pedicled and mobilized and quite often, this can’t be decided until the fat pockets are exposed... also if you have very thin skin and muscle, the transposed fat pedicle invariably will create a lumpy appearance at least for a while, thereby creating an unnecessary stress for both the patient and Surgeon...also if you are going to perform fat grafting (which in itself takes lots of finesse to the peri-orbital area) why even bother with fat repositioning and vice versa. I understand there are some Surgeons out there who yield amazing results with fat repositioning procedure, but the patients need to understand that creating an adequate fat pedicle that can be mobilized and repositioned into good position is an extremely finnesful procedure that very few Surgeons can perform well.. I hope you can find the right Surgeon with the wisdom and experience to improve your issues.. Good Luck
