There is no cream or any other product that you could place on the skin of your cheeks to reduce chubby cheeks. This issue with chubby cheeks is not the skin, but the tissue below the skin and anything you place on top of the skin will not penetrate deep enough to have any effect.
I doubt that there is any doctor on this forum who would prescribe a cream that can reduce your chubby cheeks. There may be surgical options available.

These can be removed if your cheeks are too full. You have to be careful though, as we age the mid-face gets narrower and there is often significant loss of facial fat. So, many surgeons are reluctant to perform buccal fat reduction in patients much younger than their late 20's.
