Hi! I’m 28. I’ve always had a very lean face and body with and am very active so naturally my face lacks volume. Genetically I have deeper set eyes as well. I’ve tried under eye and cheek fillers while they may help I’m not crazy about fillers and don’t want to do more. I don’t need as perfect as the after photos but looking for help to see if a lower blepharoplasty or some other procedure could help me achieve more volume in my face. It always looks worse with flash photos.

There are likely some changes in your appearance over time due to skin exposure, aging, etc., but likely the most critical factor is your baseline anatomy and genetics have you set up for this appearance. You have lost (or maybe never had) a smooth lid-cheek junction. There is some prominence of the tear trough, perhaps some herniation of the lower lid fat pad (hard to tell) and certainly some lack of mid-facial volume. Filler can be used in young patients who are good candidates but like you already noted, it is not always perfect and has to be redone. In my practice, I think the perfect approach to this situation is fat grafting with or without lower lid (trans-conj) blepharoplasty. This approach can remove excess fat (if needed) in the lower lid, and can replace fat in the mid-face and help blend the cheek again. Hope this helps! Answer: Animated face/photos It is very difficult to see details on the animated (smiling) face.
