As others have stated, the problem with photos while smiling is that the static position of the cheeks can't be accurately evaluated. Secondly, an in person consultation would be far preferable. However, as a 28 year old woman, it would be best to utilize the least invasive methods first. I would guess that a sufficient amount of Sculptra or Voluma (with the former being my preference) would be my first choice to build up your cheeks. The tear trough is a normal anatomic finding and remains one of the most over-treated areas in the face, and the one area I dissolve filler most frequently.

The best approach south either benfst grafting which give’s a longer lasting and potentially smoother result than filler. It is also possible that a lower blepharoplasty with fat grafting Would achieve a smoother result by releasing the arcus marginalis and orbicularis muscle allowing a smoother blending of orbital fat and cheek fat and softening the tear trough depression and with the addition of fat smoothing out the hollowed appearance. The best option woukd best be determined by an in person examination.

Thank you for your pictures and questions. There are likely some changes in your appearance over time due to skin exposure, aging, etc., but likely the most critical factor is your baseline anatomy and genetics have you set up for this appearance. You have lost (or maybe never had) a smooth lid-cheek junction. There is some prominence of the tear trough, perhaps some herniation of the lower lid fat pad (hard to tell) and certainly some lack of mid-facial volume. Filler can be used in young patients who are good candidates but like you already noted, it is not always perfect and has to be redone. In my practice, I think the perfect approach to this situation is fat grafting with or without lower lid (trans-conj) blepharoplasty. This approach can remove excess fat (if needed) in the lower lid, and can replace fat in the mid-face and help blend the cheek again. Hope this helps!

It is very difficult to see details on the animated (smiling) face. Whether small volume augmentation by means of fat grafts with or without lower blepharoplasty, would benefit your case can be decided only on the basis of in-person consultation, possibly teleconference. Good luck.

An in person consultation would be necessary to determine the appropriate procedure. During the blepharoplasty consultation, I examine your eyes, the presence of fat, and how much loose skin or muscle is present. In some patients who are younger, at times, a blepharoplasty can be more appropriate, because genetically we sometimes have fat pads. Good luck!
