Just wondering how a nipple more lateral than the other would be fixed, would a different size implant need to be used

A nipple that sits more lateral than the other can sometimes be improved, but the method depends on whether the issue is the nipple position, breast mound position, chest wall asymmetry, or implant pocket. Implants alone do not reliably move the nipple to the center.
If the nipple-areola is truly off to the side, a lift or areolar repositioning may be needed. If the breast mound or implant pocket is the issue, pocket adjustment and implant selection may help. I would be cautious about planning this without measurements and front-facing photos or an in-person exam.

Asymmetry of the nipple position is common, and even normal.  It really is not an issue unless it bothers the patient.  In general, the implant size will not change the lateral position of the nipple much.  Surgically, you have to talk to your plastic surgeon to see if the trade-off of extra scars is the best approach.

An in person evaluation would be needed to answer. I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants, if interested. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. it is hard to tell for sure without an examination. Generally speaking, minor asymmetry can be improved using different size implants. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.

A nipple that appears more lateral (toward the outer side of the breast) can be caused by natural breast asymmetry, chest wall shape or differences in tissue distribution.

If you are considering augmentation, implant pocket dissection can sometimes be adjusted to help center the breast mound more appropriately on the chest. In certain cases, minor tissue reshaping or selective tightening during a lift procedure may improve nipple positioning.

It’s important to understand that nipple position is influenced by the overall breast shape and footprint. Correction is typically achieved by adjusting the surrounding breast structure rather than moving the nipple independently (unless a formal lift is being performed).

An in-person consultation allows for careful measurements and surgical planning to determine the safest and most predictable correction.
